Job Description
About The Role
- Book or reschedule appointments based on callers requests.
- Handle any other hotline administrative functions such as handling of emails referral requests and emergency activations.
- Engagement of residents and community partners to address enquiries about population health and services and initiatives and link up with the appropriate team / service where appropriate.
- Support the navigation and provision of community based resources / services to support residents and caregivers to take charge of own health.
- Conduct outbound nudges to encourage enrolment and participation in screening and vaccination.
- Proactive team player to contribute to service improvement initiatives and projects and be part of implementation.
What We Are Looking For
- Recognised Diploma in any disciplines.
- Minimum 1 year of related experience in healthcare or service industry.
- Proficient in MS Office good planning and organization skills.
- Possess strong communication and interpersonal skills.
- Willing to work on rotating shifts including PHs and weekends.
