Erie Family Health Centers a nationally recognized top workplace with 13 sites in Chicago and suburbs is looking for a valuable addition to our Care Management Team! The Licensed Care Manager ensures the delivery of comprehensive highquality patientcentered care management services for Erie Family Health Centers (Eries) high risk patients. Key responsibilities include conducting facetoface and telephonic patient interactions completing risk assessments developing care plans to directly impact patient outcomes helping patients learn to manage their conditions and providing needed psychosocial resources for a panel of 75 patients.

*Compensation is based on each candidates experience skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills experience and education will be reflected in the compensation offered.

Main Duties & Responsibilities

  • Provides care management services and monthly followups for a panel of 75 patients consistent with contractual and Erie requirements for quality and service excellence.
  • Collaborates with patient programs supervisors providers and care teams as well as uses Epic reports to identify appropriate patients for care management and identify internal support programs and services.
  • Works with patient and patients care team to develop document and monitor each patients individualized treatment care plan.
  • Conducts chart reviews utilizes electronic medical records conducts brief and more comprehensive health risk assessments motivational interviewing and medication reconciliation to identify patients clinical concerns and acuity risk factors and barriers to achieving plan goals.

Qualifications

Education

  • Minimum Associates degree with 3 years nursing experience required
  • Current Illinois RN or LPN licensurerequired

Skills and Knowledge

  • Bilingual fluency (English/Spanish) preferred. (May be required to assist or work with nonEnglish/Spanish only speaking patients and/or guests.
  • Strong computer skills: expert in Microsoft Office Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system etc..
  • Active CPR card required
  • 1 year experience as a licensed professional preferred
  • Experience working in community health or safety net settings preferred
  • Medical or mental health case management experience preferred
