  • Suffolk County Council Peripatetic across West Suffolk (Community Based)
  • 27711 per annum (pro rata for part time)
  • 37 hours per week (part time working considered)
  • Permanent

This advert is open to current Suffolk County Council staff only.

An exciting opportunity has arisen to join Suffolk County Councils Healthy Living Service within the Community Health Team in Children and Young Peoples Service working in the West of the county.

The Healthy Living Service works with children and young people aged 019 25 for SEND) offering evidencebased support to children young people and their families where the child/ young person has been identified as overweight. Our service works in conjunction with the Voluntary Community Faith and Social Enterprise sector to empower families to achieving a healthier weight and lifestyle for long term outcomes.

Your role and responsibilities

As a Healthy Living Practitioner you will:

  • ensure delivery of the Healthy Living Service to all identified children young people and their families starting in the antenatal period
  • identify early children young people and families where additional preventative evidencebased interventions could reduce their risks and improve future health and wellbeing
  • to work alongside children young people parents and families to bring about sustainable improvement
  • support the whole family in maintaining their own healthy living for healthier longterm outcomes
  • provide information and signposting for children young people and families to relevant services in the local area and beyond empowering families to help themselves
  • maintain accurate uptodate records using electronic devices and systems in accordance with local policy and guidelines providing reports as required
  • to contribute to ensuring the successful completion of Quality Assurance processes within the team
  • undertake appropriate referrals to partner agencies
  • work with the community and service users to obtain user feedback and involvement to improve service delivery ensuring that the voices of underrepresented groups in the community are involved and consulted
  • respond to needs and gaps analyses and research findings by developing and implementing solutions.

You will need

  • proven and relevant knowledge and the ability to advise and support families on a range of Healthy Living related subjects
  • proven and relevant experience of working directly with children and young people and their families in a professional or voluntary capacity
  • knowledge of the social and emotional factors that affect a childs and/or young persons capacity to learn and develop
  • good working IT skills to work across a range of software.
