What Youll Do:
The Care Manager supports an assigned caseload of BH/IDD Tailored Plan members including assessment of member needs linking members to network and community resources developing and monitoring plans of care participating with care teams and addressing the barriers which may be interfering with the members desired outcomes for a communityinvolved quality life.

Education AND Experience
- A bachelors degree in a field related to health psychology sociology social work nursing or another relevant human services area or licensure as a registered nurse (RN)
- Two years of experience working directly with individuals with behavioral health conditions
- Valid Drivers license in state of residency.
