Overview
The Community Health Worker works closely with the care team and is responsible for providing assistance to UnityPoint Clinic patients. Assists patients in identifying and accessing community based resources. Provide resource education referral and followup to necessary resources to ensure optimal health outcomes.
Responsibilities
- Promotes positive customer relations in dealing with patients visitors and other employees.
- Interacts with clients to promote a full range of health services; determines the benefit of each health program after assessing clients willingness to enroll in available programs.
- Provide cultural and linguistic health education information and current resource materials to individuals and families.
- Utilize a holistic approach (considering physical psych/social spiritual educational safety and related criteria) appropriate to the age of the clients served in the assigned service area.
- Refers families to clinical social and financial support using an established algorithm to assess patient and family needs.
- Assist patients with accessing communitybased resources by supporting them with applications transportation needs and follow up on any referrals made to ensure patient utilizes identified community based resources.
- Advocate for individual and community health.
- Enhance patients ability to effectively communicate with healthcare providers.
- Works with staff and providers to create a care plan and communicates with them regarding the status of referrals and outreach efforts.
Qualifications
- High school diploma or equivalent.
- Strong understanding of community based resources.
- Experience as a direct care worker preferred
- Familiar with the VNS Community Resource Directory and/or 211 preferred
- Valid drivers license when driving any vehicle for workrelated reasons.
