Ambulatory Care Pharmacist Pharmacy Services
Part Time 48 Hours Per Pay Period Day Shift

Position Summary:
Provides innovative highquality care as an integral member of our quality improvement team. Responsible for the promotion of rational safe and costeffective drug therapy in collaboration with both internal and external primary care providers and specialists nurse care managers and other nonclinical roles. Identifies and helps resolve medication therapy opportunities while codesigning individualized therapeutic regimens for optimal management of chronic disease states and preventative care. Interfaces virtually or telephonically with our members and their caregivers to deliver Medication Therapy Management (MTM) services and help drive better individual and population health outcomes while preventing avoidable ED visits and hospitalizations.
Minimum Education:
Licensed pharmacist in the state of Tennessee; completion of a PGY1 Pharmacy Practice residency. PGY2 Ambulatory Care Pharmacy residency or a minimum of two 2 years of experience as a clinical pharmacist in an ambulatory care setting preferred.
Minimum Experience:
One 2 to three 3 years of experience in a clinical setting. Must be PC proficient with working knowledge of Windows Word and Excel.
Licensure Requirement:
Current state pharmacy license
