We are seeking a dedicated and experienced Registered Nurse (RN) to join our Home Health Agency. The ideal candidate will play a key role in initiating new cases utilizing OASIS (Outcome and Assessment Information Set) and demonstrating a comprehensive understanding of Medicare Home Health regulations. The RN will be responsible for delivering highquality patientcentered care in the comfort of patients homes ensuring compliance with all relevant healthcare standards.
Responsibilities:
- Case Initiation:
- Start and manage new cases by conducting thorough assessments of patients health status and developing individualized care plans.
- Collaborate with healthcare professionals to ensure a seamless transition of care from hospital to home.
- OASIS Utilization:
- Proficiently use OASIS to document patient assessments outcomes and interventions accurately.
- Ensure compliance with OASIS documentation standards and maintain uptodate patient records.
- Medicare Home Health Compliance:
- Stay current on Medicare Home Health regulations and guidelines.
- Implement and adhere to Medicare requirements for documentation billing and quality assurance.
- PatientCentered Care:
- Provide compassionate and evidencebased nursing care to patients in their homes.
- Educate patients and their families on healthcare procedures medications and overall wellness.
- Collaboration and Communication:
- Work closely with interdisciplinary teams including physicians therapists and support staff.
- Communicate effectively with patients families and caregivers to ensure understanding and compliance with care plans.
Qualifications:
- Active and unrestricted Registered Nurse (RN) license in the state of Virginia.
- Minimum of 2 years of nursing experience with a focus on home health.
- Proficient in OASIS documentation.
- Strong understanding of Medicare Home Health regulations and compliance.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
