Interim HealthCare of Oklahoma City is seeking a detailoriented and compassionateLicensed Practical Nurse (LPN)to join our team as aHome Health Clinical Coordinator. In this dynamic role youll help manage and support the delivery of exceptional home health care services while remaining available for timely sameday patient visits when needed.
Youll be instrumental in ensuring a smooth care journey for our patientssupporting clinical excellence promoting continuity of care and stepping in with confidence when sameday visits are necessary to support patient needs.
What Youll Do:
- Perform inhome patient visits including initial assessments followups and urgent sameday visits as needed
- Collaborate with RN Case Managers physicians and the interdisciplinary team to coordinate care and ensure patient needs are met
- Provide clinical oversight and coordination for assigned patients including scheduling and documentation
- Participate in daily team meetings and contribute to care planning and prioritization
- Maintain accurate timely and thorough documentation in compliance with agency policies and home health regulations
- Serve as a supportive clinical resource for field staff
What Were Looking For:
- Licensed Practical Nurse (LPN) in the state of Oklahoma in good standing
- Minimum of 1 year of experience in a clinical settinghome health experience strongly preferred
- Excellent assessment critical thinking and time management skills
- Strong communication and interpersonal abilities
- Flexibility to complete sameday home visits when required
- A heart for home care and a desire to positively impact the lives of patients and their families

Pay
- $25$28/hour depending on experience.
