Job Description

The Hospice House LNA delivers age appropriate quality of life care to residents in facility settings. The Hospice House LNA will effectively interact with residents family members facility staff and other healthcare team members. Per Diem Position.

* Eligibility employed 20 hours or more a week

** Eligibility employed 30 hours or more a week


Essential Functions
Follows care plan in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking bathing shaving mouth care grooming eating transfers and management of incontinence.
Communicates with nurses involved in the residents care plan; observes records and reports changes in residents condition status or other pertinent information to the nursing staff.
Does laundry housekeeping and maintenance to assure a safe orderly clean and comfortable environment.
Provides supervision to residents with cognitive deficit.
Ensures residents have the opportunity to engage in cultural spiritual physical political social and intelligent activities as the residents desire.
Maintains calm rational manner when dealing with others even in situations involving conflict or crisis.
Documents care provided to the resident accurately in accordance to the agencys guidelines.
Works independently on individual assignments yet support the needs of the total work unit.
Complies with the Agencys documentation time entry scheduling requirements
Communicates clearly and courteously with others including client family members and other agency staff.
Practicing safe work habits.
Attending Agency meetings and inservice programs.

Other Job Functions
Provides assistance and support to volunteers
Respects volunteers as an integral part of the Hospice Team
Performs other duties as assigned.

Requirements
  • High school diploma/GED accompanied by current New Hampshire LNA license
  • Ability to meet all PostOffer Preemployment requirements (including physical capacity exam background checks etc.
  • If hired be available MondayFriday for the first weeks of employment to complete the required training.
