OVERVIEW OF PRIMARY RESPONSIBILITES:
The Assertive Community Treatment (ACT) Assistant Program Director is a critical leadership position at The Guidance Center of Westchester (TGCW). Established in 2018 TGCWs ACT program is an evidencedbased practice that offers treatment rehabilitation and support services using a personcentered recoverybased approach to individuals that have been diagnosed with serious mental illness (SMI). Typically individuals served by ACT have a serious and persistent psychiatric disorder and a treatment history that has been characterized by frequent use of psychiatric hospitalization and emergency rooms involvement with the criminal justice system alcohol/substance abuse and lack of engagement in traditional outpatient services.
ACT services are provided by a mobile multidisciplinary team who meet at the program participants residence hospitals public spaces and in mutually agreed upon settings in the community that meet the participants needs. ACT supports recovery through a highly individualized approach that provides participants with the tools to obtain and maintain housing employment relationships and relief from symptoms and medication side effects. The nature and intensity of ACT services are developed through the personcentered service planning process and adjusted through the process of daily team meetings.
The goal of ACT services is to assist individuals to achieve their personally meaningful goals and life roles. ACT integrates the principles of cultural competence addressing the impact of discrimination/stigma and intersystem collaboration into its service philosophy. An essential aspect of ACT is recognizing the importance of family communitybased and faithbased supports.
PRIMARY FUNCTIONS:
  • Support the ACT Director in ensuring a highperforming culture aligned with agency values and mission and contribute to the team spirit positivity and collaboration in care delivery to program participants.
  • Provide program and clinical leadership including supervision of interns and 24 staff.
  • Responsible for a small caseload of program participants.
  • Promote support and monitor staff development in evidencebased practices inclusive of the ACT model ensuring that ACT integrates best practices and achieves its goals and objectives.
  • Supervise and/or conduct intakes and complete admissions documentation in accordance with guidelines.
  • Complete and/or ensure the completion of all documentation of contacts with program participants in electronic medical record and CAIRS system adhering to regulatory compliance clinical appropriateness participant goal(s) and billing needs.
  • Review and sign off on documentation in electronic medical records for supervisees (and other team members when needed) to ensure regulatory compliance clinical appropriateness participant goal(s) and billing needs.
  • Support ACT Director with developing/maintain protocols workflows and policy and procedure manuals to ensure that the program meets all regulations guidelines and quality assurance requirements.
  • Support ACT Director with developing/maintaining systems to monitor participant care on regular basis and proactively address concerns.
  • In absence of ACT Director lead team meetings and clinical case conferences and provide supervisory and management support to all staff
  • Function as Family Specialist on the team in order to comply with OMH requirements
  • Spend approximately 50 of time providing direct services to participants.
  • Take oncall shifts and provide support to oncall staff as needed especially related to crisis intervention.
  • Performs other duties as needed to ensure the success of the program.

QUALIFICATIONS AND ATTRIBUTES:
  • LCSW or LMHC required.
  • Positive attitude and demeanor with an ability to engage individuals of diverse backgrounds perspectives and needs.
  • Organized solutionfocused team player with leadership qualities that support a collaborative and supportive interdisciplinary team approach.
  • Demonstrated passion and commitment to supporting the wellbeing and recovery process of individuals and families struggling with complex challenges related to severe and persistent mental illness and/or cooccurring conditions such as substance use and chronic medical issues.
  • Ability to conceptualize and implement whole person care integrated medical mental health addiction and other social determinants of health needs.
  • Experience with electronic medical record documentation.
  • Bilingual Spanish speaking a plus.
EDUCATION AND EXPERIENCE:
  • Masters degree in Social Work or Mental Health Counseling
  • Minimum one year of supervisory experience preferred
