Job Description

Reporting to theProgramDirector the Patient CareManageris a member of the management team within the program/clinical area and is responsible for the coordination and direction of theunitas well as the human and fiscal resources of the unit. The Patient Care Manager collaborates with theProgramDirector Medical Director Educator and other allied health members to ensure that high quality effective and efficient services and care is provided. The Patient Care Manager is accountable for planning implementing and integrating plans that are in alignment with the Medicine Program and alignment with the organizations vision and strategic directions.

What must you have

  • Completion of Baccalaureate Degree in Nursing orother regulated health professionalfield. Masters preferred.
  • Nurse Certificate of Competence from the College of Nurses of Ontariooranother regulated Health Professional College

What else do you bring

  • Minimum of 2 5 years clinical frontline leadership experience mental health preferred.
  • Demonstrates knowledge in clinical practice.
  • Demonstrates excellence in interpersonal communication coaching mentoring and team building skills that facilitate positive interactions among all members of the health care team.
  • Demonstrates a clear understanding of the application of the RHPA legislation.
  • Demonstrates superior understanding of systems systems analysis techniques value analysis process and continuous improvement principles and tools.
  • Possesses highly developed analytical problem solving critical thinking skills.
  • Demonstrates a strong patient and customer focused philosophy in all interactions.
  • Possesses the ability to introduce and effectively facilitate change.
  • Demonstrates superior decisionmaking and organizational skills.
  • Demonstrates commitment to professional development.
  • Demonstrates ability to provide leadership support and critical analysis in stressful complex clinical and emotional situations.
  • Ability to analyze research and implement evidentbased practice into Policy and Procedure development and clinical practice.
  • Demonstrates excellence in written and oral communication.
  • Experience with guiding staff experiencing performance and/or attendance problems.
  • Communicates effectively with the multidisciplinary team and external areas using timely appropriate written verbal and nonverbal means.
  • Utilizes effective listening skills to maximize understanding of issues.
  • Provides a comfortable respectful confidential environment for team members to discuss issues.
  • Facilitates an environment that expects team members to resolve issues in a respectful nonconfrontational manner.
  • Enhanced negotiating skills.
  • Enhanced knowledge and expertise in coaching facilitation listening group and team skills.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Proven attendance record.
