Job Description
Overview
This role is responsible for collaborating with the care team in identifying assessing and coordinating resources to address Social Drivers of Health and resource barriers. VBCI (ValueBased Care Institute) Population Health BSW team members research and engage with local and state resources to determine availability and appropriateness of resources to meet patient care needs including but not limited to: food insecurity housing utilities social connections physical activity and financial barriers. The BSW works closely with patients families and caregivers to educate and provide close and persistent support will connecting patients to needed resources to remove barriers impacting the patients health and wellbeing.
Responsibilities
Participates in quality improvement initiative to help raise the standard of care across their department and the organization as a whole informing others within social work about best organizational practices.
Admits and discharges patients checking that they have the correct documentation and know their next steps to best ease their path through and outside of the organization.
Manages the social work and care plan for various patients to collaboratively create a pathway for their treatment both inside and outside of the organization.
Follows appropriate social work regulations staying up to date with potential changes to allow for the organization to not fall behind on state national or other forms of regulation related to social work.
Performs other duties as assigned.
Qualifications
EDUCATION:
Required: Bachelors in Social Work
EXPERIENCE:
Preferred: 1 year longterm care experience
