Job Description

Summary:

The provision of these job responsibilities are mandated by the Centers for Medicare & Medicaid (CMS) for Medicareapproved transplant centers. As a member of the transplant team the social worker must possess specialized clinical knowledge and advanced clinical skills to help in the areas of assessment treatment of medical patient/ caregiver navigation mental health and behavioral disorders and discharge planning.

Responsibilities:

Design and facilitate support groups and other programs.Attend multidisciplinary rounds as mandated by CMS for all Medicareapproved transplant centers and identifies appropriate resources for financial insurance transportation housing healthcare mental health spiritual services and education about diagnosis and transplantation.Develop behavioral agreements with patient and/or families that addresses adherence and commitment to treatment plan. Assist with patient fundraisingAssist patient in the comprehension of complex instructions resolve conflicts or facilities conflict resolution have strong organization/ priority setting skills must be able to learn and apply large amount of technical and procedural information must be able to multitask and must be able to communicate effectively verbally and in writing.May supervise and/or train new staff students and interns.Interviews and assesses patients and/or family caregivers and/or legal representatives. Performs comprehensive psychosocial assessments for pre post and living donors to determine transplant candidacy and continue to work with patient and family through all phases of the transplant process during inpatient and outpatient and end of life. Determines prioritizes provides and/or arranges for needed internal and external services/interventions.Provides crisis intervention for patients and family members.Participation in transplant selection committee meetingsConsults and collaborates with multidisciplinary transplant providers as mandated by CMS for all Medicareapproved transplant centers to promote monitor and evaluate patients ability to understand and adhere to treatment plan and lifestyle changes. Make appropriate referrals to community agencies such as DCF APS SSA CMS disease specific resources as deem necessary.

Additional Job Information:

Complexity of Work: Requires critical thinking skills effective (verbal and written) communication skills decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful and fast paced environment and take appropriate action. Must have strong organizational/priority setting/multitasking skills. Must be able to learn and apply large amounts of technical and procedural information.Required Work Experience: Minimum of two 2 years post MSW experience in related field required. Previous experience working in hospital setting.Other Information: Ongoing clinical transplant education required. Participation in Quality Assurance and Performance Improvement (QAPI).
