HANAC Inc. originally founded by George Douris as the Hellenic American Neighborhood Action Committee is a New York Citybased multifaceted social services organization. The organization was founded in 1972 to serve the needs of vulnerable populations throughout New York City.
The Tenant Coordinator will provide social services for tenants in HPDs Senior Affordable Rental Apartments (SARA) Program in Corona Queens. The tenant coordinator will work closely with the Program Director Social Work Supervisor and another Tenant Coordinator to develop and utilize best practices to assist tenants in 67 apartment units ages 60 some of whom were previously homeless.The Tenant Coordinator will be responsible for all aspects of case assistance including ongoing service plans managing and following through with referrals developing relationships with collateral agency providers and assisting clients with remaining permanently housed. The Tenant Coordinator will also be responsible for working with the Program Director and other Tenant Coordinator to implement programming and activities that create a positive inclusive community environment.

Responsibilities:
- Maintain regular contact with tenants on caseload to assess their needs and assist where needed
- Develop individual care plans in coordination with the tenants goals and update a minimum of every 6 months
- Coordinate case management needs including applying/maintaining entitlements and benefits healthcare coordination when necessary and assistance with maintaining housing
- Maintain good rapport with Property Management in order to best assist tenants when issues arise
- Build relationships with community providers; provide advocacy and resource linkages/referrals as required
- Work with service providers and family members to build networks of support
- Plan activities and events to foster a sense of community among tenants including celebrations community provider presentations training arts and crafts bingo etc.
- Completion of Initial Intake assessment and file consent form in a timely manner when new tenants move in
- Conduct unit inspections with Property Management annually and periodic wellness checks when needed
- Maintain daily case assistance log complete detailed and accurate progress notes in a timely manner complete detailed monthly report for HPD in a timely manner and attend required training
- Assist Program Director as needed in preparation for biannual audits
Qualifications and Experience:
- Bachelors Degree in Social Work or related field;
- Knowledgeable in benefits and entitlements for lowincome senior residents including Section 8 Medicaid and SNAP benefits.
- Microsoft Office proficient
- Bilingual preferred Spanish Chinese or Korean preferred
- Must be peopleorientated professional and courteous with excellent phone etiquette.
- Must be flexible friendly responsible and reliable. Able to work independently but also part of a small team. Ideal candidate will be able to handle an emergency if one arises and implement problem solving skills
