SUMMARY
The Los Angeles County Department of Health Services Correctional Health Services
(DHSCHS) is currently seeking a highly motivated and dedicated individual to fulfill the role of Substance Use Disorder Community Ambassador. In this position you will be tasked with developing and executing an impactful reentry linkage program that encompasses patient navigation and peer support services for individuals transitioning from correctional facilities back into the community.
The Staff AnalystHealth (SAH) role entails serving as a technical expert and consultant to management regarding the provision of case management and medication assisted treatment services postrelease. The SA will conduct thorough analyses and provide recommendations for optimizing resource utilization program implementation and operations for CHS Addiction Medicine Services. Additionally the SA will be responsible for undertaking special assignments and projects as assigned.

ESSENTIAL FUNCTIONS
- Develop and implement a comprehensive reentry linkage program to support individuals in accessing addiction medicine services and other necessary resources upon release from correctional facilities.
- Provide patient navigation services to assist individuals in navigating the healthcare system and accessing necessary addiction medicine services.
- Organize and facilitate peer support groups to provide emotional support and encouragement for individuals in recovery.
- Collaborate with community partners and stakeholders to develop and implement reentry support services tailored to the specific needs of the population served.
- Create and develop workflows for postrelease services such as medical services counseling services recovery or emergency housing transportation services appointment services and education services.
- Provide support and assistance to individuals in accessing health and behavioral healthcare benefits including helping with enrollment in insurance programs and navigating the healthcare system.
- Collaborate with community partners and agencies to ensure a smooth transition for individuals postrelease including coordinating referrals and followup services.
- Conduct assessments and evaluations of individuals needs and develop individualized treatment plans in conjunction with the healthcare team.
- Monitor and assess the effectiveness of services provided and make recommendations for improvements as necessary.
- Maintain accurate and detailed records of services provided outcomes achieved and progress towards goals.
- Act as a liaison between individuals postrelease and the healthcare team advocating for their needs and ensuring they receive necessary support and services.
- Participate in meetings trainings and other activities to support the goals and objectives of the Department of Health Services Correctional Health Services.
JOB QUALIFICATIONS
- Bachelors degree in public health social work or a related field. Masters degree Preferred.
- Experience in data analysis community engagement or program support in correctional health services and addiction medicine settings.
- Strong organizational skills with attention to detail and ability to manage multiple tasks simultaneously.
- Excellent interpersonal and communication skills to effectively engage with diverse stakeholders and community members.
- Knowledge of addiction treatment principles recovery support services and community resources in Los Angeles County.
- Commitment to promoting health equity and improving outcomes for individuals with addiction issues in the local community.
Education/Experience
- Four years of experience in a staff capacity analyzing and making recommendations for the solution of problems of organization/program.
Certificates/Licenses/Clearances
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out jobrelated essential functions.
- Successfully clear the LA County Live Scan process.
- Successfully clear the Health Clearance
- Must comply with LA County Health Officer COVID19 vaccine order. Must be Fully Vaccinated.
