- Works under the supervision of the Program Director
- Collaborates with the interdisciplinary team to create personalized care plans based on the patients unique needs and goals of care.
- Participates in clinical huddles and case conferences with team members such as physicians nurses social workers speech therapists and occupational therapists
- Conducts physiotherapy assessment consultation and planned interventions for patients living with a lifelimiting illness
- Engages in skilled compassionate communication with patients families and caregivers regarding mobility function and endoflife care goals
- Educates and mentors patients family and caregivers on strategies to maintain safe mobility and function in the home setting
- Maintains accurate and timely documentation of patient assessments treatment plans and outcomes ensuring adherence to ethical and legal standards
Qualifications :
- Masters degree in Physiotherapy
- Registered in good standing with the College of Physiotherapists of Ontario
- 35 years of experience working as an Physiotherapist preferred
- Valid Ontario drivers license and a reliable vehicle
- Ability to work autonomously and as part of an integrated health care team with an emphasis on collaborative care planning
- Expertise in managing complex physical function and mobility issues in palliative care an asset
- Proficiency in conducting assessments interpreting findings and formulating evidencebased treatment plans.
- Familiarity with assistive devices adaptive equipment and environmental modifications
- Excellent communication and interpersonal skills
- Strong clinical skills and a passion for improving the quality of life for patients and their families
- Effective problem solving and critical thinking abilities
- Proficiency in computer software programs (e.g. Microsoft Office) and virtual communication platforms (e.g. Microsoft Teams Zoom Go To Meeting)
