As we continue to expand we are looking for dedicated Nurse Practitioners to join our esteemed team.
Position Overview:We are seeking skilled and compassionate Nurse Practitioners to conduct home visits in New Yorks boroughs Suffolk and Nassau Counties. This role offers the opportunity to make a meaningful impact on patient care while enjoying a flexible work environment.
Key Responsibilities:
- Conduct comprehensive assessments and evaluations of patients in their homes.
- Develop and implement personalized care plans based on patient needs.
- Manage and stabilize chronic and acute medical conditions.
- Provide highquality medical care and support to patients and their families.
- Collaborate with other healthcare professionals to ensure optimal patient outcomes.
- Maintain accurate and timely documentation of patient interactions.
Qualifications:
- Valid Nurse Practitioner license in New York State.
- Board certification is required.
- Must be enrolled as a Medicaid provider.
- Previous experience in home healthcare or similar settings preferred.
- Strong clinical assessment and decisionmaking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
Compensation:
- Competitive rate: Earn up to $200 per visit (rate based on visit type).
- This exclusive rate is available through December 31 2024.
