    We are looking for a fulltime permanent Intensive Case Manager to join our team!

    Reporting to the Program Manager Outreach the Outreach Intensive Case Manager will provide case management supports to the homeless population in Peel Region in particular identifying people who are homeless or at risk of homelessness and engaging them in services that support their challenges with mental illness or mental health and substance use.

    Status: Permanent fulltime 37.5 hours per week evening hours and weekends may be required; Note this posting is for an active vacancy

    Salary: $61201..19 annually. (Candidate qualifications will be considered when determining the starting salary)

    Location: This position is based out of our 60 West Drive Brampton office. Due to the nature of this program this role is onsite and/or community based.

    What You Will Do:

    • Support the transition of persons into housing utilizing a critical time intervention approach to housingbased street outreach.
    • Conduct needs assessments develop and implement comprehensive care plans and ongoing monitoring.
    • Provide contracted case management taskbased interventions and activities of daily living related to social determinants of health.
    • Utilize multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed.
    • Manage a caseload including regular reassessments updating risk levels and caseload reviews.
    • Participate in regular staff meetings and care planning to provide case management expertise for client care within multidisciplinary team.
    • Facilitate groups including the creation of new groups as needed and acting as a backup facilitator.
    • Contact with primary care to support clients care plan and discharge summary as required.
    • Collaborate with other Case Managers to support program needs.
    • Update all appropriate documentation within 24 hours of client contact as required by CMHA PD policy and procedure.
    • Develop and facilitate other groups or social recreation activities as required by the program.

    What You Need to Join Our Team:

    • Postsecondary degree or diploma in a human services field.
    • Minimum 2 years experience working with vulnerable populations. Experience with case management in a community setting required.
      • Demonstrated knowledge of serious mental illness concurrent disorders and/or dual diagnosis is required.
      • Knowledge of Critical Time Intervention (CTI) is an asset.
      • Knowledge of Housing First Principals is an asset.
      • Comprehensive knowledge of Recovery Principles.
      • Empathize and connect with those experiencing homelessness and the mental health and addictions system.
      • Build rapport and initiate conversation with others using interpersonal skills.
      • Approach client care nonjudgmentally.
      • Utilize trauma informed best practices.
      • Strong interpersonal skills with proven ability to approach listen build trust and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence.
      • Openness to operate from an antiracist antioppressive 2SLGBTQ affirming and intersectional framework.
      • Willingness to learn and apply practical understanding of different elements of diversity such as gender identity/expression race/cultural background religion sexual orientation disability immigration and precarious status family status etc.
      • Fluency in a second language including ASL (American Sign Language) is considered an asset.
      • Valid Canadian drivers license and access to an insured automobile in good repair.
      • A satisfactory Vulnerable Sector Screening (Police Check) report.
