Leadership Responsibilities:
Develop and oversee patient care plans ensuring the highest standards of care.
Be a mentor and coach! Train support and guide staff to help them excel in their roles.
Lead performance reviews to recognize and enhance staff competency.
Monitor key performance indicators (KPIs) to ensure care quality and client satisfaction.
Drive innovation by implementing improvement initiatives based on client feedback and data.
Expand our reach by building relationships with healthcare providers and referral sources.
Clinical & Care Management:
Ensure personalized care by completing clinical assessments for Medicaid clients.
Coordinate seamless patient care by making appropriate referrals for services.
Maintain compliance with all state and federal regulations licensing and certification requirements.
Advocate for clients by addressing concerns requests and feedback with empathy and action.
Be the bridge between clients and caregivers ensuring smooth communication and care coordination.
Engage with the community through events seminars and health fairs to promote our mission.
Training & Staff Development:
Lead by example and mentor peers and staff to uphold professional nursing standards.
Empower caregivers and staff through handson training and ongoing development programs.
Evaluate skills provide constructive feedback and celebrate successes.
Ensure safety and best practices are consistently followed across all care settings.
Develop emergency response plans to protect and support clients during crises.
What Were Looking For:
Bachelors degree in Nursing (Active RN License required)
57 years of leadership experience in home care healthcare administration or a related field.
Handson experience in personal care services hospice or residential care programs.
Strong knowledge of home care regulations and best practices.
Excellent communication leadership and problemsolving skills.
A compassionate clientfocused approach with a commitment to quality care.
Proficiency in healthcare management software and electronic health records.
Ability to stay calm under pressure and make critical decisions with confidence.
