What Youll Do

NOTE:Please know this position will need to work in Everett and Renton (few days of the week)

The SSVF Healthcare Navigator / Case manager will workclosely with Homeless and Formerly Homeless Veterans and their Families and is part of the YWCAs Specialized and Integrated Services Division. TheSSVF Healthcare Navigator works closely with Veterans to assist them incommunicating their preferences in care and personal healthrelated goals tofacilitate shared decision making of the Veterans care. The SSVF HealthCare Navigator serves as a resource for education and support for Veteransand families and helps identify appropriate and credible resources andsupport tailored to the needs and desires of the Veteran.

In addition to healthcare navigation services SSVF Healthcare Navigator / Case managers will identify and engage with current and previouslyhomeless families to assist them in obtaining and retaining permanentaffordable housing. This will require establishing effective case managementrelationships with formerly homeless veterans and homeless veteranfamilies building relationships with property managers and other socialservice providers in coordination with the SSVF Program Manager I.

This position assists in connecting families to supportive services andhealthcare services to help ensure success in permanent housing. Thisposition requires the SSVF Healthcare Navigator/Case Manager to conductassessments on all veterans enrolled within the program. This requiresworking with the SSVF team to ensure that they are part of the enrollmentprocess for all clients being brought into the SSVF program. In addition thisposition will be expected to respond to crisis situations which could result inloss of permanent housing if not resolved. This position has a caseload offamilies seeking housing as well as families experiencing difficulty inmaintaining housing.

Responsibilities

    • Conduct Healthcare assessments and determine program eligibility upon enrollment into the SSVFprogram.
    • Understand the VA and outside medical healthcare systems.
    • Be knowledgeable and build relationships within the medical mental rehabilitation and treatment centers.
    • Ensure veterans are reminded of their healthcare appointments.
    • Provide referral and connection to supportive services such aschemical dependency treatment mental health counseling domesticviolence advocacy etc.
    • Provide outreach tolocate identify engage and refer homeless and formerly homelessveterans and their families to benefits and services.
    • Provide links to housing and housing stability resources.
    • Build relationships with landlords/property managers.
    • Work in collaboration with other social service providers in thecommunity.
    • Assess the strengths and challenges of the client and assist in developing housing stability goals individual goal plansand action steps.
    • Provide or ensure provision of life skills training and support whenneeded.
    • Conduct home visits to ensure housing stability.
    • Accurately and thoroughly document performance of all tasks.
    • Meet regularly with SSVF team to help ensure success for the familiesand the program.
    • Carry a caseload between 1825 of veterans who need housing as wellas provide healthcare navigation for all clients enrolled into SSVF.
    • Attend meetings with human service providers and other publicfunctions as related to the SSVF program.
    • Help with administrative support of the program through regularreporting and record keeping as directed.
    • Show demonstrated ability to interact with people of different culturesand promote social justice.

Requirements

    • BA in social services or related field and/or a minimum three years experience working with lowincome individuals and families and or persons with disabilities with a strong focus on supportive services and healthcare navigation for Veterans.
    • Knowledge and experience in providing services to Veterans.
    • Experience working with homeless families/individuals and knowledgeable of family dynamics.
    • Experience with crises intervention and case management.
    • Experience working with persons who have chemical dependency and mental health issues.
    • Experience with working multiple healthcare systems including the VA.
    • Knowledge of and experience working with persons who have experienced domestic violence.
    • Knowledge of housing resources in King County Landlord/Tenant Laws and Fair Housing Regulations in Seattle and King County. Ability and willingness to work as part of team and independently.
    • Ability to perform Data Entry in HMIS database.
    • Values diversity and provides culturally competent services.
    • Must be able to travel independently throughout King and Snohomish County.
