Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester Putnam and Ulster Counties. A pioneer Federally Qualified Health Center Open Door provides more than 300000 patient visits annually and serves nearly 60000 individual patients who might not otherwise have access. Founded as a free clinic in 1972 Open Doors mission of building healthier communities through accessible equitable culturally competent health care has led to site and service expansions.
Today Open Door operates centers in Brewster Mamaroneck Mount Kisco Ossining Port Chester and Sleepy Hollow in addition to eight SchoolBased Health Centers in the Port Chester and Ossining School Districts a mobile dental van in Mount Kisco and a new dental practice in Saugerties NY. Open Door is accredited by the Joint Commission and is recognized by the National Committee on Quality Assurance as a PatientCentered Medical Home. Open Door achieves clinical results that consistently surpass national benchmarks for patient outcomes.
Open Door is seeking a full time Nurse Practitioner or Physician Assistant for its Mamaroneck location. The NP or PA will report to the site Medical Director. He/she will lead a crossfunctional team in the delivery of patient care.
Responsibilities
Provide high quality primary care in medical home setting to Open Door patients of neonatal pediatric adolescent adult and geriatric groups.
Lead a crossfunctional care team.
Provide comprehensive patient care and establish a family partnership whenever possible to ensure the needs of the patients are met on an ongoing basis.
Ensure high quality primary is delivered costeffectively consistently.
Provide appropriate specialist referrals followup and hospitalization when indicated.
Maintain electronic medical records in compliance with Open Doors standards with high efficiency.
Remain accountable for hospitalized patients.
Engage patient family and crossfunctional care teams in sustained health and wellbeing of the patient.
Partner with Open Doors Behavioral Health team and resources
Maintain familiarity with ongoing research and development in relevant field
Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions.
Represent Open Door in community outreach activities
Requirements
Degree and licensure to practice in New York as a Family Nurse Practitioner or Physician Assistant.
Proficiency in Spanish preferred.
A minimum of three years relevant experience. Communitybased outpatient experience is highly desirable.
Outstanding bedside manners: patientcentric with empathy excellent active listening and communication skills engaging and efficient.
Outstanding clinical capabilities and judgment.
Passionate about providing high quality primary health care to lowincome underserved residents and those lacking access to health care.
Focused on collaborative teambased care.
Responsive and capable of working in a high accountability environment.
Comfort level with using an electronic health record.
Flexible.
Excellent judgment and ability to prioritize resources where they will be most useful.
Comfortable receiving and providing constructive feedback.
Engaged in learning and continuous quality improvement.
Outstanding written and verbal communications skills with a culturally diverse patient and staff population.
