Job Description
Project Transitionis looking for a bachelors levelPsychiatric Rehabilitation Counselor.
Primary responsibilities include:
Psychiatric Rehabilitation Counselorto provide support to adults with intellectual disabilities and behavioral health needs. Responsibilities include: helping to develop implement and update treatment/recovery plans for assigned members. The PRC also provides individual and group services to the targeted population.
Qualifications:
Bachelors degree with a major in chemical dependency psychology. social work counseling or nursing (with a clinical specialty in human services); advanced fluency in ASL; and strong interpersonal skills.
Primary responsibilities include:
- Admission/orientation of new members
- Participate in the treatment/recovery planning process with assigned members
- Treatment Planning and Updates
- Provide documented individual support coaching and rehabilitation counseling for assigned members
- Prepare and provide skills group and workshops with follow up documentation
- Supporting members to complete Distress Tolerance Plans
- Orient member and family to phone coaching system
- Other duties as assigned by Program Director
