Job Description

The DHS Complex Case Service Coordinator will provide assessment outreach linkage and assist with the acquisition and coordination of resources and services for the identified clients on their caseload. This individual will be responsible for administering the Child and Adolescent Needs and Strengths Assessment (CANS) to assist in the service planning for the youth and/or their parent/guardian. Additionally the Complex Case Service Coordinator will provide ongoing support for youth placed in specialized group homes which currently include Devereux Mapleton Family Links Phoenix Pathways and Horizon Keystone Pathways Enhanced (Oil City) and Family Links shelter (for youth who do not have current SC assignments). These programs are subject to change based on additions or deletions to the DHS contracted provider list. The CANS is a multipurpose tool developed to support care planning and level of care decisionmaking to facilitate quality improvement initiatives and to allow for the monitoring of outcomes. The assessor will facilitate communication across childserving agencies and better identify the needs and strengths of youth and their families to support and maintainthe youths path toward recovery. Work is performed with considerable degree of initiative and independent judgment in accordance with established regulations policies and procedures. The position reports directly to the supervisor who provides general guidance regarding program issues.

DUTIES AND RESPONSIBILITIES
  1. Maintain CANS credentialing on a yearly basis
  2. Meet all timelines identified by the Department of Human Services screening process
  3. Maintain a caseload of 2530 youth
  4. Completes case notes and documentation within the DHS documentation system
  5. Completes the CANS assessment in the Key Information Demographic System (KIDS)
  6. Implement the CANS screening tool for referred eligible youth placed in care
  7. Ability to travel to identified contracted providers as needed
  8. Attend and participate in County Integrated Childrens Planning meetings
  9. Complies with and adheres to HIPAA confidentiality policy
  10. Coordinates with families providers Office of Children Youth and Families juvenile probation educational system and any other community resources as necessary to facilitate appropriate services and resources
  11. Attends and participates in personal development and remains competent and knowledgeable of current trends initiatives resources and services available for clients and families


SKILLS AND KNOWLEDGE

  5. Ability to be a selfstarter and creative in identifying solutions to problems; knowing when to inform supervisor when obstacles arise
  6. Knowledge of Microsoft Office products including Word Excel and PowerPoint
  7. Ability to adapt to and learn new software products
  8. Ability to work with people from a variety of diverse agencies backgrounds resources and communities

ESSENTIAL REQUIREMENTS
  1. Possess Act 33 Act 34 and FBI clearances
  2. Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
  3. Have current auto insurance that meets minimum collision coverage
  4. Knowledge of guidelines and regulations of all levels of service coordination
  5. Strong oral and written communication skills
  6. Excellent leadership qualities
  7. Ability to work independently
  8. Fulltime position requiring 40 hours per week

EDUCATION AND EXPERIENCE

Masters Degree in Social Work Education or Psychology or RN Degree or a Bachelor Degree in Social Work Education or Psychology with two years experience in the CASSP system.
