Responsible for the evaluation clinical assessment treatment plan implementation and progression of goaldirected purposeful Occupational Therapy interventions. Responsibilities also include consultations with medical staff and health professionals completion of medical records and insurance forms and supervision of Rehab associates.

MINIMUM QUALIFICATIONS:

EDUCATION CERTIFICATION AND/OR LICENSURE:

1. Graduate from a school of occupational therapy.

2. Current Occupational Therapy license within the state of West Virginia or appropriate state where services will be provided..

3. Valid drivers license.

4. CPR certification within 30 days of hire date.

PREFRRED QUALIFICATION:

EDUCATION CERTIFICATION AND/OR LICENSURE:

1. Registered by the American Occupational Therapy Certification Board.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an allinclusive list of all responsibilities and duties. Other duties may be assigned.

1. Complies with ethical and professional standards of conduct as established by the American Occupational Therapy Association.

2. Prescribes and instructs patients in therapeutic selfcare and creative activities that promote functional independence with attention to both physical and mental components.

3. Performs evaluation procedures as per Policy and Procedure: selfcare assessment FIM assessment ROM manual muscle test upper extremity/hand assignment sensory testing interest checklist splint/orth assessment position assessment and vital signs.

4. Educates patient and/or responsible party to home programs and the use of assistive devices. Assesses learning and provides reinforcement or implements further teaching strategies as indicated per policy and procedure.

5. Provides care based on physical psychosocial communication safety and education level and related criteria for the infant pediatric adolescent adult geriatric patient per the established agespecific standard.

6. Evaluates home environment for concerns plan of treatment.

7. Compliance with all Home Health/ standards including but not limited to visits and communication.

8. Completes plan of treatment and recertification as per Medicare guidelines. Completes SOC Recertification and Discharge per guidelines.

9. Performs CPR when directed by the patients/appropriate representatives wishes.

10. Displays safe efficient and prudent use and maintenance of D.M.E. and medical supplies and adaptive equipment.

11. Familiar with supply requisition systems as per policy and procedure.

12. Performs treatments/procedures according to specific standards.

13. Performs and monitors patient teaching according to the policy and procedures.

14. Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard.

15. Identifies the stages of death and dying.

PHYSICAL REQUIREMENTS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

1. Must be able to lift move and position patients of all weights with varying amounts of assistance required in a fair safe manner on a frequent basis.

2. Must be able to lift 50 pounds occasionally 25 pounds frequently and 10 pounds courteously.

3. Must be able to walk stand stoop kneel reach push/pull on a frequent basis.

4. Must be able to handle/grasp objects with the ability to perceive attributes such as size shape temperature and texture.

1. Exposure to bodily fluids human tissues contagious diseases sharps and explosive gases.

2. Exposure to toxic gases fumes and odors.

7. Exposure to wind rain snow sleet mud high and low temperatures animals and insects and hazardous roadways.

8.The majority of work is performed in the patients home and may require extensive travel.

9. Subject to unpredictable situations. May be exposed to adverse or unpleasant home environments.

SKILLS AND ABILITIES:

1. Must have strong communication skills written and verbal.

2. Must be able to problem solve in an efficient manner.

3. Must be able to initiate selfdirected learning/research of information within the field of Occupational Therapy and other health care disciplines

4. Demonstrates knowledge of and compliance with safety policies and procedures.
