Job Description
***We offer a $3000 signing BONUS***Salary range is $120000$145000 per year DOE
As a Family Practice Nurse Practitioner/ Physician Assistant you will be an integral part of our team responsible for providing patient centered healthcare to individuals and families inclusive of health status age gender identity and sexual orientation focusing on health promotion and disease prevention in our integrated health clinic.
The Family Nurse Practitioner/ Physician Assistant supports the Prisma Community Care mission of providing affirming and inclusive services to promote wellbeing and advance health equity for diverse communities and all those seeking compassionate care especially people of color 2SLGBTQIA and Queer individuals and those affected by HIV.
What your day will look like at Prisma Community Care:
- You will provide quality general internal medicine family medicine and pediatric medical care to patients including preventative & primary care HIV medication management GAHT prescribing/monitoring and PrEP prescribing/monitoring by following the most current evidencebased guideline and procedure of practice.
- You will function independently to perform ageappropriate medical history and develop an appropriate assessment and plan of care in the direct management of patients with acute and chronic illness and for health maintenance/screening exams.
- Your work productivity must demonstrate the ability to see a minimum case load of 2 patients per hour average 36 patientfacing hour & 4 admin hours per week); effectively work patients into schedules as caseload demands and respond to a variety of unforeseen daily circumstances including emergency care of patients.
- You will collaborate with multidisciplinary team members by making appropriate internal/external referrals to behavioral health Prevention Specialists and medical case management.
- You will participate in Prisma Community Cares Integrated Care Team meetings to promote integration of patient care to foster optimum patient outcomes.
- You will build a patient base by networking marketing & communicating with partner agencies and the community to meet the goal of a full panel.
- You will represent the Wellness program in local/national media and community events that promote awareness and utilization of Prisma Community Cares services.
- You will participate in development of internal and external marketing materials to educate and promote wellness services offered.

Who were looking for:
- You possess a current and valid Arizona license from the State Board of Nursing for Advanced Nursing Practice OR the Arizona Regulatory Board of Physician Assistants.
- You meet all clinical obligations of the medical practice: accrediting agency compliance CLIA compliance drug prescription compliance including DEA certification HIPAA state and federal regulations safety regulations continuing education and training to keep abreast of current developments in the industry etc.
- You are credentialed with AHCCCS and major health insurance plans.
- You have the ability to establish and maintain effective working relationships with patients their families and staff members and agency leadership.
- You are a highly organized selfstarter with excellent problemsolving skills and ability to multitask effectively.
- You have a minimum of 1 year experience in Family Medicine.
- You have excellent written and verbal communication.
- You have the ability to work in a fastpaced environment with changing deadlines and priorities while maintaining a positive attitude.
- You are proficient in Microsoft Office (Word Excel Outlook PowerPoint) software applications and competent in using electronic medical records and information systems in providing effective documentation of patient care.
- Youve demonstrated cultural competency to professionally and respectfully serve the 2SLGBTQIA communities.

- You have 2 years of Family Medicine experience and a strong understanding of HIV disease and HIV related primary care PrEP/PEP and GAHT.
- Youve earned your AAHIVM accreditation.
- Experience serving the health needs of Lesbian Gay Bisexual and Transgender communities.
- You are bilingual in Spanish and English.
Environmental Factors Physical Requirements and Compliance
- Work primarily in a climatecontrolled environment with minimal safety/health hazard potential.
- Office environment exposure to computer screens for lengthy periods of time.
- May require evening and occasional weekend work.
- While performing the duties of this job the employee is regularly required to walk sit stand; use hands talk and hear. The employee is occasionally required to reach with hands and arms. Specific vision abilities required by this job include close vision distance vision and ability to adjust focus.
- This position may require the employee to occasionally climb balance stoop kneel crouch bend or crawl.
- This position must frequently lift and/or move up to 10 pounds and occasionally lift and/or move more than 100 pounds and assist clients.
- Currently have or be able to obtain within 90 days of employment a valid Fingerprint Clearance Card
- Currently have or are able to obtain within 7 days of employment a clear TB test
- Currently have or are able to obtain within 7 days of employment a current flu vaccination
- Currently have or are able to initiate within 7 days of employment a HepatitisB primary vaccination series
- Currently have or are able to obtain within 14 days of employment a CPR certification
- Currently have or be able to obtain within 1year of employment a AAHIVM accreditation/certification
