Job Title: Social Worker Hospital Discharge Liverpool
Location: Liverpool
Pay Rate:43.78 per hour
Liquid Personnel is recruiting a Social Worker for the Adults Hospital Discharge Team in Liverpool. Candidates must have SWE registration UK work eligibility a degree in Social Work and postqualified experience.
The role involves conducting assessments and developing support plans for individuals being discharged from mental health hospitals. It requires working within a multidisciplinary team (MDT) to ensure coordinated care safeguarding individuals by identifying and managing risks and supporting recovery and independence through personcentred planning. Additionally the role includes collaborating with partner agencies to facilitate safe and effective discharges.
