Job Description

Encompass Support Services Society is an accredited nonprofit agency that provides a wide range of programs and support services in Langley and Aldergrove for over 25 years. The Opportunity
Foundry is removing barriers and increasing access to health and wellness services for young people aged 1224 and their families through a network of youthfriendly centers across British Columbia and online. By offering integrated mental health care substance use services primary and sexual health care youth and family peer support and social services. Foundry makes it easier for young people to find support in their communities. Online resources and firstofitskind virtual care further broaden Foundrys reach.

Reporting to the Foundry Langley Medical Director the Family Physician along with the primary care team provides episodic enhanced and longitudinal services. They must be able or willing to train to support genderaffirming care opioid agonist therapy and contraceptive support. Interventions may be provided on a 1:1 basis to families on outreach and to group. Flexibility in hours and availability outside of typical office hours will be required to support dropin hours and potential outreach.

SALARY RANGE: In accordance with the Ministry of Health negotiated clinical service contract.

Key Duties and Responsibilities:
Provides services assessment triage and support for youth in the areas of mental health sexual health physical health and substance use;
Provides proactive client care by using EMR supports and quality improvement initiatives and maximizes use and scope of interdisciplinary team members;
Monitors ongoing care orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment
Provide care and guidance to patients when it comes to taking medication as well as what side effects they may have and any interactions that may take place with medication the patient is already taking;
Establishes priorities for management of health diseases disorders and conditions; provides followup treatment; communicates with patients and families (as required) about health findings diagnoses and priorities outcomes and prognoses; supports and counsels patients/clients/residents in their responses to diseases disorders and conditions;
Performing physical exams including pelvic and pap exams if indicated;
Supporting sexual health care requisitions contraception labs etc.;
May provide genderaffirming care including assessment resources referrals etc.;
May provide addiction care involving medically assisted treatment (MAT) such as Opioid Agonist Treatment (suboxone methadone /kadian for opioid use disorder or Naltrexone for alcohol use disorder;
May provide Medical Detox screening and withdrawal management;
May provide medications or other treatments to reverse or mitigate the effects of opioid ;
Collaborate with mental health and substance use services including emergency mental health MHSU ACT teams Youth Detox communitybased substance use services Urgent ShortTerm Treatment and Assessment MCFD and other youthserving community partners;
Collaborates and consults with physicians nurse practitioners or other health care and social service providers as appropriate to assess and diagnose patient status;
May participate in research contributing to improved patient care and advances in primary care health policy development and population health.
Maintains youth population health focus (ages 12 24 by implementing screening and health promotion activities for youth populations at risk.
May participate in interdisciplinary Foundry Langley staff education through case presentations mentoring role modeling and facilitating the exchange of knowledge; fosters health care partnerships.
Develops implements and evaluates policies and procedures related to primary care interdisciplinary care and health system practices.
Participates in team meetings case conferences and organizational initiatives;
Provides traumainformed antioppressive genderaffirming decolonial and antiracist principles of care;
Uses a harm reduction approach to care;
May take part in quality improvement initiatives to improve client outcomes including projects across acute and community care that support service improvements for clients;
May provide learning opportunities and support for students and residents attached to the teams;
Performs other duties as required/assigned.


Qualifications:
1. Education and Knowledge
A medical degree and eligibility for licensure in good standing with the College of Physicians and Surgeons of British Columbia.


2. Training and Experience
Eligibility for a faculty appointment with University of British Columbias Faculty of Medicine is an asset.
Enthusiasm for and experience working with young adults and adolescents is required.


3. Skills and Abilities

Demonstrated knowledge and skills in the following areas:
Reproductive health including sexual health and STI prevention and treatment;
Common physical and emotional problems of young people;
Child and adolescent physical psychological and social development;
Harm reduction strategies for young people;
Health promotion and prevention strategies;
GenderAffirming Care;
Awareness of and respect for diverse cultural beliefs and practices;
The unique ethical legal and confidentiality issues in caring for young people; and
Hospital and community resources for young people.
Excellent interpersonal communication skills with the ability to work collaboratively in an interdisciplinary team (primary care mental health substance use peer support and social services);
Exceptional problemsolving skills and the ability to help others to use the same analytical thought processes; and
Ability to critically assess and evaluate health research literature to determine best practices and the ability to introduce education and evidencebased research;
Ability to assess and recognize population health trends;
Ability to plan and implement strategies for population based on prevention and health promotion;
Ability to implement and evaluate planned change;
Ability to transfer knowledge as appropriate;
Ability to identify and respond appropriately to legal and ethical issues that may arise in patient care;
Ability to selfassess performance and assume responsibility and accountability for own professional development educational or consultative assistance when appropriate;
Empathic and compassionate with good listening skills and creative thinking;
Proficient in comprehensive screening and assessment practices using established standardized measures and approaches;
Knowledge and experience working with vulnerable youth population;
Knowledge of systemic issues and risk factors facing underrepresented groups including 2SLGBTQIA and Indigenous youth and young adults;
Knowledge and respect of the diversity of the BC population;
Demonstrated problemsolving and decisionmaking skills ability to gather and process information to support the organization and its team;
Demonstrated knowledge and application of traumainformed antioppressive decolonial antiracist and genderaffirming principles of care;
Providing patientcentered care by:
empowering and advocating for patients and their decisions and choices and
incorporating patients values beliefs and cultural practices in care plans and service deliver.
Effective listening and consensusbuilding skills that promote communication and a cooperative approach to problemsolving and decisionmaking.
Ability to work effectively with others in a multicultural environment independently and as part of a team including understanding the frameworks of other disciplines.
High degree of selfawareness and capacity to apply and maintain professional integrity boundaries and confidentiality;
