We are seeking a Facilitator to act as a liaison and advocate for children and NonMinor Dependents (NMDs) placed in our ShortTerm Residential Therapeutic Program (STRTP). This position will coordinate and facilitate the Child and Family Team (CFT) process in a manner that ensures comprehensive collaboration communication and treatment planning to and on behalf of each child/NMD within the STRTP.
This is a nonexempt position that reports directly to the Assistant Director of Mental Health Services.
ESSENTIAL FUNCTIONS and Responsibilities:
Act as liaison between children/NMDs within the STRTP family members/identified supports service providers DCFS personnel and the community for the purposes of providing case management advocacy needed resource information and linkage and highly coordinated services as directed by the Child and Family Team (CFT).
Adhere to Los Angeles County Department of Mental Health (LACDMH) and Wayfinders Mental Health Policy and Procedures for the billing and documentation practices related to any and all services provided under Early Periodic Screening Diagnostic and Treatment (EPSDT) funding and maintain minimum productivity expectations.
Ensure that the principles of the Core Practice Model are adhered to by all team members including the identification of client and family strengths the providing of services in a timely and appropriate manner and being the primary contact point for children families service providers and community resource personnel.
Demonstrate an ability to work professionally and independently with limited supervision as well as an ability to exercise professional discretion and independent judgment based on training education experience and personal creativity.
Demonstrate professional and effective communication abilities including the completion of written reports letters and plans and initiating and responding to verbal communications with clients families the CFT and inter and intraagency personnel.
Qualifications:
Masters degree in Psychology Social Work Counseling or related field plus two 2 years of experience in a mental health setting as a specialist in the fields of physical restoration social adjustment or vocational adjustment OR
Bachelors degree in Psychology Social Work Counseling or related field plus four 4 years of experience in a mental health setting as a specialist in the fields of physical restoration social adjustment or vocational adjustment OR
Associates degree plus six 6 years of experience in a mental health setting as a specialist in the fields of physical restoration social adjustment or vocational adjustment required.
Experience working highrisk children adolescents adults and/or families within the Wraparound Mental Health Educational Child Welfare and/or Juvenile Probation systems required.
Bilingual: (English/Spanish) preferred.
Must meet eligibility requirements to bill for MediCal services.
