Job Description

BronxWorks helps individuals and families improve their economic and social wellbeing. From toddlers to seniors we feed shelter teach and support our neighbors to build a stronger community. RESPONSIBILITIES

  • Conduct onsite outreach and advertising for planned events
  • Conduct home visits to clients as needed and actively engage clients who are absent or decrease their participation level and involvement in the services
  • Develop and maintain connections with internal and external agencies and companies who can provide services and programs to clients
  • Make ongoing assessments on what interests clients have
  • Liaise with case management staff to collaborate with them on clients programming needs and concerns and share relevant information to the case managers.
  • Maintain records of clients interactions in electronic database
  • Complete reports on a monthly basis or more frequently as required
  • Maintain familiarity with agency and city resources available to clients
  • Document progress notes for all encounters and services provided to clients
  • Complete assessments for all clients monitor clients progress towards goals documentinteractions with or on behalf of clients according to program requirements
  • Participate in regular supervision with supervisor
  • Perform additional duties as assigned

Program Specific Responsibilities

  • Become Soar Certified within one month of hire and assist with completing applications forSSI/SSD
  • Plan and facilitate onsite and offsite informational sessions and teaching programming about employment support and educational support
  • Coordinating variety of activities groups and events that cater to all ages of clients including children teenagers and adults
  • Provide crisis interventions and manage emergency situations appropriately
  • Maintain and ensure accuracy of records and documentation of all client interactions Create thePeer Program paperwork training manual presentation and work flow of application process
  • Facilitate training session for the Peers and provide ongoing support and meetings with all Peers in program and all host sites
  • Network internally and externally for further employment and educational opportunities for clients
  • Collaborate with CQI staff case management and clinical staff to coordinate and run various events groups and activities
  • Work one Saturday shift per month

QUALIFICATIONS

  • Proficiency in modem business communications including personal computers electronic mailvoicemail facsimile and copier equipment
  • Associates degree preferably in the social services (Social Work Psychology Sociology HumanServices etc. and 2 or more years of experience in case management or employment services

PHYSICAL REQUIREMENTS

  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand walk or sit for long periods of time.
  • Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work.
