As an Integrated Chronic Pain Therapist you will be an integral part of a collaborative interdisciplinary team focused on delivering seamless services and programs to our clients and customers. What youll be doing:
- Conduct comprehensive assessments of patients pain experiences psychological factors and treatment needs
- Educate patients and families on chronic pain the brains role and cognitiveemotional reprocessing
- Provide individual and group therapy using Pain Reprocessing Therapy (PRT) and other mindbody approaches
- Develop and adjust individualized treatment plans integrating cognitivebehavioral and other therapeutic modalities
- Collaborate with the care team which includes therapists physicians physiotherapists and other practitioners to ensure holistic coordinated care
- Monitor document and communicate patient progress updating treatment goals as needed
- Facilitate case conferences team discussions family meetings and discharge planning
- Maintain accurate and timely records of patient interactions assessments treatment plans and progress notes in compliance with Homewood and College standards
- Support continuity of care by liaising with referral sources and coordinating outpatient services
- Participate in policy development program planning and quality improvement initiatives
- Stay current on latest research and best practices engage in continuing education and contribute to team learning
- Participate in quality initiatives; tracks and monitors appropriate indicators
- Actively maintain a healthy team culture by demonstrating active engagement openness and respect in interactions with colleagues at all levels of the organization
What were looking for:
- Masters degree in Social Work Psychology Occupational Therapy Physical Therapy or a related healthcare discipline
- Valid registration with the appropriate professional college or regulatory body in the province
- Two years or more experience providing Psychotherapy
- Specialized training and/or certification in Pain Reprocessing Therapy (PRT) is strongly preferred: in the absence of this training it will be provided
- Additional training in CognitiveBehavioral Therapy (CBT) Somatic Experiencing MindfulnessBased Stress Reduction (MBSR) or related modalities is preferred
- Strong clinical assessment and treatment planning skills with the ability to manage complex multifactorial chronic pain cases
- Exceptional communication skills with the ability to collaborate effectively with patients families and multidisciplinary teams
- Empathetic and patientcentered approach
- Demonstrated ability to establish trust and provide holistic care that addresses the emotional physical and psychological components of chronic pain
- Ability to adapt treatment plans based on individual patient needs and evolving evidencebased practices
- Experience facilitating group therapy sessions and educational workshops
- Demonstrated success working collaboratively as part of an interdisciplinary team
- Awareness and acknowledgement of the value of using measurementbased care when supporting patients/clients to help guide clinical decision making; experience an asset
- Ongoing commitment to professional development in chronic pain management and integrated therapy practices
