Location: Multiple locations based around burg VA 24501
Position Summary:
The Case Manager serves as a support coordinator ensuring that services to individuals with developmental disabilities and intellectual disabilities are planned individualized and personcentered. The Case Manager provides services that may include assistance to individuals and their family members in accessing needed services that are responsive to the individuals needs.
Required Education and Experience:
Related Knowledge Skills & Abilities:
Knowledge of: Services systems and programs available in the community including primary health care support services eligibility criteria and intake processes and generic community resources and treatment programs; The nature of developmental and intellectual disabilities serious mental illness substance use disorders and/or cooccurring disorders depending on the population served including clinical and developmental issues; Different types of assessments including functional assessment (i.e. VIDES) and their uses in service planning; Treatment modalities and intervention techniques such as behavior management independent living skills training supportive counseling community integration supported employment family education crisis intervention discharge/transition planning and service coordination; Types of developmental mental health and substance use services available in the locality; The service planning process and major components of a service plan; The use of medications in the care or treatment of the population served; and all applicable federal and state laws and regulations and local ordinances.
Skills in: Identifying and documenting an individuals need for resources services and other supports; Using information from assessments evaluations observation and interviews to develop individual service plans; Identifying services and resources within the community and established service systems to meet the individuals needs and documenting how resources services and natural supports such as family can be utilized to achieve an individuals personal habilitative/rehabilitative and life goals; and coordinating the provision of services by public and private providers.
Ability to: Work as team members maintaining effective inter and intraagency working relationships; Work independently performing position duties under general supervision; and engage in and sustain ongoing relationships with individuals receiving services; Complete the DBHDS core competencybased curriculum within 30 days of hire; Participate in and successfully pass behavioral intervention training which includes both verbal deescalation skills and physical skills techniques to be utilized with consumers as needed; Drive and must possess a valid Virginia drivers license. May be called upon to travel to any Horizon worksite; transport clients provide services to clients at other locations. Regular attendance at training and Horizon meetings that may require travel to a variety of Horizon sites; Willing and available to work modified schedules (weekends holidays etc. as required.
