Position Summary:

Lakes Region VNA and Hospice is seeking a compassionate and dedicated Social Worker to join our team. The Social Worker will support both Hospice and Home Care patients and their families. This role involves assessing the psychosocial needs of patients developing care plans and offering emotional support to help patients and their families navigate the complexities of healthcare. The ideal candidate will have a masters degree in social work and at least one year of healthcare experience.

Key Responsibilities:

  • Psychosocial Assessment:Conduct comprehensive assessments of patients psychosocial needs strengths and challenges.Identifynecessary interventions and resources to support the patient and family.
  • Care Plan Development:Collaborate with the interdisciplinary team to create and implement individualized care plans. Ensure that social emotional and financial needs are addressed.
  • Patient and Family Support:Provide counseling and support to patients and their families to help them cope with illness loss and endoflife issues. Offer guidance on advanced care planning and decisionmaking.
  • Resource Coordination:Assistpatients and families in accessing community resources including financialassistance support groups and other services. Facilitate referrals toappropriate agenciesand organizations.
  • Crisis Intervention:Respond to emergency situations and provide crisis intervention as needed. Address urgent needs related to safety housing and mental health.
  • Documentation:Maintainaccurateandtimelydocumentation of all interactions and care plans in the patients medical record. Ensure compliance with regulatory and organizational standards.
  • Team Collaboration:Participatein interdisciplinary team meetings to discuss patient care and progress. Work closely with nurses physicians and other healthcare professionals to ensure comprehensive care.
  • Education and Advocacy:Educate patients and families on their rights and available services. Advocate for the patients needs within the healthcare system.
  • EndofLife Care:Provide specialized support to hospice patients and their families focusing on comfort dignity and quality of life.Assistin the bereavement process as needed.

Qualifications:

  • Education:Bachelors Degree at minimumMasters Degree in Social Work (MSW) from an accredited institutionpreferred.
  • Experience:Minimum of one year of experience in healthcare preferably in a hospice or home care setting.
  • Licensure:Current and valid Social Worker license in the state of New Hampshire.
  • Skills:
    • Effective communicationand interpersonal skills.
    • Ability to work effectively with diverse populations.
    • Excellent organizational and time management skills.
    • Knowledge of community resources and social services.
    • Proficient in electronic medical records (EMR) and documentation.

Working Conditions:

  • Environment:Work will be conducted in patients homes hospice care settings and the community.
  • Physical Requirements:Must be able to travel within the service area perform tasks involving physical activity and manage stressful situations.
  • Schedule: Parttime or Per Diem with occasional oncall responsibilities.
    • 3 days 2024hrs/week
    • On call rotation is every 4th weekend 8am4pm Sat and Sun
