Job Description
The Glycemic Management Advanced Practice Provider will serve as a full time Advanced Practice Provider to provide care to patients at SUNY Upstate Downtown and Community Campus. The Advance Practice Provider will provide inpatient glycemic management of the diabetic patient. Responsibilities include providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients ordering appropriate laboratory testing & diagnostic studies arranging for referrals testing consultation & therapeutic services and maintaining outstanding relationships with families referring physicians & all members of the health care team.
NYS licensed/registered NP or PA required. Masters Degree as a Nurse Practitioner or as a Physician Assistant and current National Certification consistent to the clinical practice also required. CPRBLS Certification required at the time of appointment.
Monday Friday Days with possible weekend rotation.
