Physical Therapist (PT)

Salary and Benefits
- Fulltime benefitseligible role with a paypervisit structurelow $60 max $67 (max is for 20 years of experience)
- Mileage Reimbursement: $0.585 per mile

Responsibilities
- Provide physical therapy services as prescribed adhering to agency policies and regulations
- Conduct initial and ongoing patient assessments including OASIS to improve mobility and independence
- Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams
- Deliver oneonone patient care in a home health setting
- Utilize EMR (Kinnser/Wellsky preferred but not required) for accurate documentation
- Participate in oncall holiday and weekend rotations as needed
- Patient assignments are grouped geographically to minimize travel
- Supportive management team and strong company culture
- Growth opportunities available as patient census increases
- PRN rates available for exceeding productivity goals (details available upon request)
- Productivity Expectations:
- 30 points per week / 60 points biweekly
- New point system breakdown:
- Start of Care Oasis (SOC) visit: 2.5 points
- Recert visit: 1.75 points
- Resumption of Care (ROC): 1.75 points
- Evaluation visit: 1.25 points
- Discharge Oasis: 1.5 points
- Discipline Discharge: 1.0 points
- Routine visit: 1.0 point
- Precepting: 0.5 points visit fee
Requirements
- Active Florida PT license
- Minimum of 1 year of home health experience preferred
- Current CPR/BLS certification
- Fulltime schedule: MondayFriday (most visits between 8 AM and 5 PM)
- Ability to cover all of Collier County including Naples Marco Island and Immokalee
