Alfred Health is a leader in health care delivery improvement research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne from ambulatory to inpatient and home and communitybased services.

The Department

The Palliative Care Service provides specialist Palliative Care input to patients of Alfred Health. Team members include consultant medical staff palliative care nurse coordinators junior medical staff grief counsellors music therapists as well as administrative support. Palliative Assessment and Treatment Service (PATS) is a dynamic fast paced clinical service providing outreaching acute palliative care to people referred to Alfred Palliative Care with life limiting illnesss with the aim of providing symptom relief and improving their quality of life safely in a range of settings.

We are looking for someone who is:

  • An experienced and organised professional with a strong commitment to enhancing health experiences for palliative patients.

  • Eager to contribute to equity in healthcare enhancing coordination and flexible service delivery models. The ideal candidate will shape their practice around the values of Alfred Health.

The Role

The Palliative Care CNC will have the following responsibilities:

  • Expert care: Provide comprehensive assessments diagnoses and tailored treatment options directly to patients whether inperson or through Telehealth
  • Collaborative planning: Work with multidisciplinary teams to create personalised care plans that prioritise patient goals and preferences
  • Empowerment through education: Offer health education and support to patients families and care staff ensuring optimal outcomes and understanding of care processes
  • Community connections: Build strong partnerships with Community Palliative Care services community services and families to manage patient care effectively
  • Adapt and lead: Evolve your practice to meet the needs of our service and take an active role in professional development and leadership within the Palliative Care team

Qualifications and Experience Required

  • Current endorsement with the Nursing and Midwifery Board of Australia via AHPRAas a Registered Nurse
  • Palliative care experience
  • Post graduate qualification in Palliative Care preferred/working towards
  • Advanced interpersonal communication (written and verbal) and problemsolving skills
  • Knowledge understanding and experience in research and quality improvement
  • Current and satisfactory Police Check
  • Current and satisfactory Working with Children Check
  • Current Victorian Drivers Licence
