We are seeing a full time Licensed Practical Nurse for our Homes in Forest Park OH
This is a full time MondayFriday day shift position 7a3p
Position Summary:
Monitor and meet the health needs of the individuals we serve in the home through evaluating recording and consulting with the physicians and staff of the home. Adhere to all Federal State and County regulations and achieves performance outcomes.
Position Accountabilities and Performance Criteria:
- Visits each individual on a regular basis to evaluate his/her immediate physical condition and to receive his/her comments relating to needs and problems; and reviews health records and medication cards. Achieves performance thresholds especially those related to medical processes.
- Ensures that all individuals we serve who are ill or have clinic appointments do not go to school workshop job or camp.
- Schedules or delegates attendance at all medical appointments and follows through with all charting and recommendations.
- Administers ordered medications and treatments to the individuals we serve or verifies that they are given based on the delegated nursing rules..
- Supervises administration of medications treatments labs menses and seizure activity. Conducts active treatment audits as assigned.
- Weighs takes vital signs and measures height of individual when assigned.
- Charts in the nursing notes any change in individuals behavior health status noting treatments and followup services.
- Completes quarterly Nursing Summary on each resident assigned.
- Documents accurately in the nursing notes so they are informative and descriptive of the supervision and care rendered.
- Trains staff on medical procedures conditions medication disease management and progress and documents accordingly.
- Assists in the care and training of the individuals we serve as prescribed in the IHP. Ensures all critical health information be documented in the IHP.
- Checks P.O. and M.A.R. (Medical Administrative Record) forms monthly for accuracy.
- Orders mediations as needed to ensure availability.
- Works with other consultants who provide services for the individuals we serve.
- Reviews the health requirements of each individual admitted to the facility and assists the attending physician in planning the individuals care. Notifies physician of status changes in accordance with standard nursing practice.
- Informs the Home Manager and family/guardian of the individuals medical status and other related matters through written reports and/or verbal communication.
- Provides Gentle Teaching interactions to individuals in our communities. Complete the five hour Gentle Teaching course level.
- Observes/communicates with the support staff to meet the delegated nursing requirements.
- Audits individual charts to ensure accuracy on a quarterly basis and achieves performance thresholds.
- Complete mandatory training as required.
- Participates in the IHP process for the individuals we serve (annually quarterly and special team meetings).
- Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
- Adhere to all policies and procedures of the organization regarding the use of technology and technologybased information.
- Contributes to achieving customer satisfaction performance thresholds.
- Responsible for welcoming and onboarding new employees.
- Performs other duties as directed.
Position Qualifications:
Minimum Qualifications: Must have a current LPN license to practice in the State of Ohio. Must have a current CPR/First Aid and Pharmacology card. Must have a valid drivers license and a good driving record.
Minimum Experience: Experience in working with persons with developmental disabilities or
dualdiagnosis with adults in a residential setting preferred. Knowledge of state and federal
regulations
