Job Description

Current licensure asa Registered Nurse in the Commonwealth of Massachusetts

Graduateof an accredited nursing program.

Bachelor of Sciencein Nursing preferred; new managers must have a BSN degree effective May 2016.

Certification inAmbulatory Nursing preferred.

BLS Certification

English and Spanishfluency required other languages desirable

Previous experiencein Ambulatory or Primary Care with Medical Homes and serving the underserved

Previous experienceas a supervisor coordinator or manager preferred.

Experienceprecepting new nurses or emerging leaders preferred.

Certified DiabetesEducator

Certified ChronicDisease Management (Stanford model or comparable training)

Specific training orexperience serving the underserved

Proficiency in allthe RN competencies required on his/her unit

Interpersonal skillsand interprofessional collaboration

Organizational(executive function) skills

Ability to speak andwrite succinctly and persuasively.

The Nurse Manager working in Ambulatory Nursing is a Registered Nurse with the oversight and management of daily operations for a clinical practice. The nurse manager is responsible for assuring competent compassionate individualized nursing care for patients is provided by the staff including but not limited to medical assistants licensed practical nurses registered nurses and nurse practitioners he/she manages. He/she is accountable for patient safety and quality of care. He/she collaborates with the practice manager and physician leader in the practice to ensure that teambased care is coordinated comprehensive and that care transitions are seamless. As part of the unit leadership team he/she is jointly responsible for operational and fiscal efficiency and effectiveness of the unit.

Principal Duties and Responsibilities:

Direct patient care within the Scope of RN no less than 20 hrs/week for reimbursable visits with patients.

  • Chronic disease manager on site overseeing clinical initiatives in chronic disease at the site and the community
  • Supports all providers in areas of disease management programs Joint Commission Safety and Quality
  • Patient Advocate in collaboration with Corporate Patient Advocate.
  • Vaccine management directly or through delegation to RN
  • Infection control in collaboration with Infection Control Director at corporate office
  • Annual employee flu vaccination
  • Tracking of Pap and all diagnostic tests
  • Phlebotomy as needed.

Management and evaluation of all clinical operations including nursing medical support and community health related services and operations

  • Design evaluate and monitor practice works to ensure service excellence operational efficiency and maximization of role groups
  • Coordinates scheduling and work systems that support adequate coverage and the smooth flow of daily clinical operations.
  • Secures resources for staff.
  • Ensures support and resources for regular huddling activities.
  • Trains and supervises staff and monitor Point of Care testing and specimen collection activities.
  • Performs timekeeper and payroll related activities.
  • Manages resources within an unit budget. Oversees a supply inventory management system. Oversees the delivery of facility administered medications.
  • Interviews recruits and makes recommendations for hiring. Conducts annual performance evaluations. Conducts competency testing as needed.

Assures staffing which provides competent compassionate patientandfamilycentered evidence based nursing care utilizing the nursing process and interdisciplinary teamwork and collaboration.

  • Utilizing their assessment critical reasoning and astute clinical judgment the nurse manager expedites the delivery and/or delegation of appropriate care and treatment to the patient
  • Regularly reviews staff documents to ensure quality and timeliness
  • Creates a collaborative environment
  • Actively empowers staff to manage their professional lives address conflict and participate in the work environment.
  • Demonstrates and mentors behaviors of respect and support to staff for patient and familys cultural beliefs and values individual needs health goals and treatment preferences.

Collaborates with all members of the health care team to ensure coordination of care.

  • Reviews and followups with members of the health care team that patients have received the necessary care.
  • In collaboration with the health care team evaluates effectiveness of care and interventions for assigned group of patients.
  • Design a system of communication with members of the health care teaminpatient and/or in the community to ensure continuity in care transitions.
  • Actively engaged in creating a healthy safe respectful work environment.

Provides leadership in the practice to ensure safe efficient and cost effective care.

  • Maximize the role of all staff
  • Actively evaluates cultural sensitivity and manages issues related to inclusivity within staff the unit and/or with patients.
  • Identifies opportunities and solutions to ensure a safe efficient and cost effective practice environment.
  • Demonstrates compliance with regulatory requirements.
  • Guide patientcentered medical home related activities.
  • Participates in quality and safety initiative.
  • Participates in operational and capital budget development process.
  • Generates innovative ideas to improve care delivery and work flow in the practice environment.
  • Precepts and mentors new members of the team and emerging leaders.
  • Participates in departmental and organizational committees and initiatives.
  • Facilitates the implementation of clinical research projects.
  • Holds regular staff meetings.
  • Leads spreads and sustains regular performance improvement activities.
  • Oversees practice registry activities related to population health efforts.
  • Annually discusses career development opportunities with staff.
  • Implements and evaluates nursing activities as part of Magnet designation.
  • Participates on committees to improve the work of the unit.
  • Provides intermittent coverage to peer practice manager or physician leader as needed.
  • Develops and implements a coverage and a succession plan.

Professional Development

  • Maintains and updates clinical knowledge and skills based on current nursing and nursing management practice.
  • Adheres to departmental requirements for required training.
  • Identifies specific learning needs and goals and collaborates with unit leadership in developing a plan to meet them.
  • Identifies and facilitates learning opportunities for her/himself and staff.
  • Participate in committees to develop new skills.
  • Regularly participates in leadership development activities
