Job Description
Responsibilities
- Conduct initial and repeat RN Assessments for our clients
- Develop Plans of Care based on the clients needs and services requested
- Supervise provision of care according to the Plan of Care and the performance of the home health aide(s)
- Review and make reasonable inquiries to determine changes in client status periodically
- Communicate with authorized family / personal representatives in the care of the client when needed
- Document all changes in the clients medical condition
