The SevereAsthma Center is a new initiative at BWH to provide state of the art evaluationand care for patients with difficult to treat asthma. Patients will include out of town and foreignpatients. The center will bring togetherpulmonologists allergists otolaryngologists psychiatrists speech therapistsand gastroenterologists. The physician assistant will be at the nexus of thisprogramassisting with program development while providing and optimizingpatient care. Duties of this positioninclude direct patient care patient (and provider) education help withprogram development performing clinical procedures such as allergy skintesting and administration of biologics and coordination of patient visits. The Physician Assistant will work closelywith physicians in the Severe Asthma Program and with other medical andadministrative staff in the Center for Chest Diseases. The selected individual will be responsiblefor the previsit assessment of patients assisting with their first clinicvisit and for seeing patients in their routine and emergency visitfollowup. There is the opportunity forcreativity in program development since the Severe Asthma Program is a newinitiative within Partners Asthma Center and BWH Lung Center.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Duties:
Clinical: 40%time)
PreVisit:
Perform previsit record evaluation withassessment and decisions regarding additional diagnostic evaluationsnecessary.
Contactpatients as necessary to review history and records and clarify additionaltesting and scheduling.
Office Visit:
Preassessmentat visit
Postvisiteducation and medication administration
Conductadditional independent patient visits in regularly scheduled clinic
Conductpatient visits for emergency followup.
Allergyskin test administration.
PostVisit:
Communicateto patients and referring providers to clarify interventions and followup andalter interventions as necessary.
NonClinical: 60%time)
Administrative:
ProgramDevelopment: Work with physician leaders of the Severe Asthma Program topromote/advertise the program to patients and providers including as neededmeeting with BWH Marketing and speaking with Partners and outside providersabout the Program.
Developsclinical bestpractice templates
Assistin developing clinical data tabulation instruments
Identifypatients appropriate for asthma research trials
Helpto coordinate specialized severe asthma treatment programs such as antiIgEmonoclonal antibody therapy antiIl5 monoclonal antibody therapy andbronchial thermoplasty.
Data Collection:
Facilitatecreation and maintenance of a Severe Asthma Program database.
Teaching:
Beavailable to teach about asthma to appropriate audiences as a representative ofthe BWH Severe Asthma Program at Partners Asthma Center.
Helpcreate/coordinate relevant teaching programs presented by staff at the SevereAsthma Program.
New Initiatives:
Workwith staff of Severe Asthma Program to develop and implement a program foroutoftown patients to undergo structured a multiday evaluation that mayinclude procedures (bronchoscopy laryngoscopy cardiopulmonary exercisetesting) and evaluation by multiple consultants.
Helpto develop a lifethreatening (nearfatal) asthma registry with institutionof a program for ongoing close medical followup and outcomes monitoring.
QUALIFICATIONS:
1. Describe the formal education theincumbent(s) must possess in order to perform the essential functions of thejob. This must be realistic &supportable on the basis of the duties and responsibilities listed in the descriptionthen preferred e.g. Bachelors required Masters preferred.
2. Describe minimum years and type ofexperience required including Orientation time then statepreferred requirements.
Each PA I shall meet thefollowing minimum requirements for work experience:
a.2 years relevant full time experience as a PA (preferablyoutpatient or in the pulmonary area)
b.Other previous Hands on patient care experience may beconsidered at the discretion of the Medical Director.
3. Listany specific license/certification/registration required to perform the job.
a.NCCPA certified
b.License as a Physician Assistant in Massachusetts
c.Maintain current DEA and Massachusetts ControlledSubstance Licenses
d.Complete 100 hours of CME according to national andstate regulations during each 2 year period of employment.
e.Maintain current BLS/ACLS certification
f.Complete BWH Professionalism Program within 3 months ofdate of hire.
g.Complete BWH Procedural Sedation Training ifdetermined necessary.
4. Specific Additional Qualifications requiredfor the PA II position (position specific information)
a.Experience in allergy/pulmonary medicine valued but notrequired.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
1. List special analytical administrativetechnical and/or financial abilities or skills required.
Each PA shall demonstrate thefollowing professional Knowledge and skills:
a.Proficient with BWH computer systems as needed toindependently perform daily tasks such as order entry EMAR lab tests andresults incident reporting BICS etc.
b.Consistently provide high quality and timelydocumentation including admission notes progress notes discharge summariesand dictations.
c.Must have knowledge of billing practices.
2. List physical and mental abilities required.
PAs:
a.Must be energetic quality driven and have provenability to be a productive member of a cohesive team of peers dedicated toproviding optimum patient care.
b.Must exercise the highest degree of professionaljudgment under the direct supervision and guidance of a licensed physician.
c.Must maintain a professional attitude towards patientsand be able to manifest mature independent originality in solving problems notcovered by existing guidelines within legal limits to meet the needs of thepatients.
d.Must maintain a professional attitude towards patientsat all times and an excellent and collegial working relationship with otherpersonnel involved in patient care.
e.Must demonstrate understanding of procedures policiesand documentation required to ensure compliance with hospital standards ofcare.
3. For Direct Patient Carepositions list skills/competencies required to provide care appropriate to theage of the patients served on his/her assigned unit.
Each PA shall demonstrate thefollowing clinical knowledge skills and competencies:
a.Demonstrate competency to effectively diagnoseformulate appropriate treatment plan and efficiently implement the plan withoutmuch direction and with a greater level of independence from the attendingphysician.
b.Clinical skills and knowledge as well asprofessionalism of the PA candidate are assessed by Severe Asthma Programfaculty under the direction of the Directors of the Severe Asthma Program.
c.Candidates with two or more years of relevant PAexperience prior to joining the Severe Asthma Program become eligible for apreliminary evaluation of competencies approximately 3 months after joining theservice. The purpose of the preliminary evaluation is to provide feedback tothe candidate on progress towards level of competency required and provideguidance on additional development needs. After approximately 6 months afterjoining the Severe Asthma Program the candidate then becomes eligible tocomplete the assessment and if successful advances to a PAII.
d.If a candidate does not pass the competency assessmentSevere Asthma Program attending MDs will provide feedback on additionaldevelopment needs. Reassessment is possible after 6 months of focuseddevelopment.
e.Competencies are assessed by Severe Asthma Programattendings either through direct observation or verbal communication. Theattending physician(s) performing the assessment will fill out a competencyassessment (designed by Severe Asthma Program directors). The candidate is responsible to provideexample cases which he or she has cared for patients with the most commondiagnosis.
WORKING CONDITIONS:
1. Describe the conditions in which the work isperformed. Consider noise dustchemicals temperature extremes etc.
Work will be performed in outpatient medical offices andadministrative offices within the Brigham and Womens Hospital.
2.Describe the level intensity and duration of exposure to any adverseworking conditions. For example: Frequently (up to 50 of work time)works in records warehouse where there is exposure to moving equipment anddust.
No exposure to any adverse working conditions isanticipated.
3. Listany hazards to the employee and others.Specify probable consequences of carelessness including the possibilitythat such carelessness could result in physical harm to the employee and/orothers.
No special hazards are involved. Medication administration and allergy skintesting involve the risk of needle sticks.
