Job Description

VNAcare a notforprofit leader in home care is looking for dedicated healthcare professionals to join our hospice team.

The Physical Therapist position is open in our Murrieta location and surrounding areas. This is a PerDiem position during our day shift.


Requirements of the Physical Therapist:

Graduate of a program of physical therapy approved by the APTA and the

Council of Medical Education of the AMA.

Current licensure for practice in the State of California.

At least two year of experience in a clinical setting.


Physical Therapist Duties Include:

Implements current physical therapy practice.

Ensures quality and safe delivery of home health and hospice care services.

Develops a Plan of Care based on the patients medical needs and discusses with the Case Manager and/or Clinical Manager. Revises plan as necessary for appropriate utilization of services and discusses changes with members of the Interdisciplinary Team.

Promotes effective written/verbal communications.

Uses equipment and supplies effectively and efficiently.

Identifies need for intraagency referrals as well as interagency referrals.

Promotes education for clients client families and the community.

Participates in personal professional growth and development.

Conferences with Case Manager and/or team members during the patients course of treatment.

Provides inservice to other staff members as requested.
