Job Description
Description of the role:
University Home Care is seeking an experienced Rnhomecare with oasis experience to join our team in Livonia Michigan. As an Rnhomecare you will be responsible for providing skilled nursing care to home care patients. You will work closely with patients and their families to develop and implement individualized care plans that promote their wellbeing and independence.
Responsibilities:
- Perform comprehensive assessments of patients health status
- Develop and implement individualized care plans
- Administer medications and treatments
- Monitor patients progress and make necessary adjustments to care plans
- Educate patients and their families about their health conditions and selfcare
- Coordinate with other healthcare professionals to ensure continuity of care
- Document and maintain accurate patient records
Requirements:
- Valid RN license
- Prior experience in home care
- Proficiency in using OASIS software
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong attention to detail
