Primary Function/General Purpose of Position

Provide professional pharmaceutical care for Roper St. Francis Healthcare inpatients and outpatients by establishing practicing and promoting high standards of quality care.

Essential Job Functions

Specialized Patient Care

  • Provide costeffective pharmaceutical care to patients being managed for specific disease states (e.g. antimicrobial therapy cardiac vascular critical care).

  • Participate in the selection of drug therapies and monitor these patients on a daily basis.

  • Discuss medication order changes/clarifications with the appropriate prescriber and

  • document any changes made.

  • Participates in related conferences committees or meetings and the P&T Committee as related to drug selection restriction and medication protocol development and review.

  • Participates in the development of initiatives related to successfully meeting JCAHO standards national patient safety goals and hospital core measures.

  • Participate in hospital quality improvement efforts.

  • CMS Inpatient and outpatient Core Measures.

  • National Patient Safety Goals

  • Promote and demonstrate rational cost effective drug therapy.

  • Review select patients medication therapy for accuracy and appropriateness.

  • Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved.

  • Monitor the use of expensive medications to assure there is use consistent with accepted or approved criteria.

  • Develop and conduct target drug programs and drug usage evaluations as needed.

  • Cost Savings Initiatives research proposal presentation implementation and followup.

  • Participate in applicable Research Studies.

General Patient Care

  • Respond to specific pharmacy consults from physicians and other health care providers Provide clinical pharmacy services to the appropriate clinical areas including (SICU CICU CVICU Cardiac Units etc).

  • Participate in multidisciplinary rounds monitor drug therapy and evaluate appropriateness of use dose dosage form regimen route therapeutic duplication and drug interactions.

  • Document Clinical Interventions.

  • Provide clinical support to the pharmacy.

  • As requested assist pharmacy staff with patientspecific problems or concerns.

  • Determine creatinine clearance to facilitate appropriate drug dosing.

  • IV to Oral conversion of : intervention reports will be monitored for possible conversion of target to the oral or NG route of administration.

  • Promote the use of the formulary by converting nonformulary orders to formulary based on health system policies and procedures.

  • Monitor the use of expensive medications to assure there is use consistent with accepted or approved criteria.

  • Monitoring of patients receiving hematologic growth factors such as CSFs and erythropoietin through established guidelines.

  • Detect monitor document and report adverse drug reactions and medication errors.

  • Perform initial consult and followup of Pharmacokinetic Service patients.

Education/InServices

  • Take a role in the teaching programs of the Department of Pharmacy Services.

  • Provide staff education to pharmacy staff nurses and physicians.

  • Provide development opportunities in the management of specific disease states for the pharmacy staff.

  • Newsletter Contributor.

  • Precept and mentor pharmacy practice and other specialty residents.

  • Services to the Profession

  • Precept Pharm. D. clerkship students.

  • Presentations at local regional and national venues.

  • Service to BPS ACCP PSAP SCSHP and other professional organizations as a reviewer committee member and/or officer.

  • Serve as a reviewer in the biomedical literature.

  • Professional Development

  • Attend relevant professional meetings and seminars.

  • Remain current with the general pharmacy and specialty literature.

  • Performs other duties and responsibilities as required.

Licensing/Certification

  • Licensed by the State Board of Pharmacy to practice the profession of pharmacy in South Carolina.

Education

  • Graduate of an accredited A.C.P.E. College of pharmacy Pharm. D. Degree Completion of a Pharmacy Practice Residency.

  • A specialized residency with experience in an area of clinical interest (Anticoagulation Critical Care Infectious Disease Cardiac Care) (preferred)

Work Experience

  • Previous hospital experience or suitable hospital experience during pharmacy education or internship (required)

  • Experience in pharmacy clinical program development and promotion (preferred)

Working Conditions/Physical Requirements

Frequent sitting. Frequently requires long periods of working at a computer. Intermittent walking standing stooping bending. May require lifting or moving items up to 25 lbs. Frequent use of finger/hand dexterity. Constant talking or hearing. Corrected hearing and vision to normal range. No exposure to blood body fluids or tissue. Possible exposure to infectious materials communicable diseases and/or other conditions common to a healthcare environment. Work is performed in patient care environment decentralized and main pharmacies. Requires work under stressful conditions deadlines and/or irregular hours. Ability to read and comprehend. Travel may be required.

Skills

Hard/Tech/Clinical Skills:

  • Knowledge of professional pharmacy practices.

  • Knowledge of hospital pharmacy medication distribution systems.

  • Possesses skills necessary to monitor medication therapies in relation to the age of the patient to include pediatrics adolescents adults and geriatrics.

  • Skill in reading and interpreting the English language.

  • Must be able to effectively supervise pharmacy supportive personnel.

Soft/Interpersonal Skills:

  • Skill in identifying problems and recommending solutions.

  • Skill in establishing and maintaining an effective working relationship with patients hospital/medical staff and the public.

  • Ability to interpret adapt and apply guidelines and procedures.

  • Ability to communicate clearly.
