Schedule: Hybrid position with 23 days on site at Virtua Voorhees location at 200 Bowman Dr.
Summary:
Serves as the primary provider of psychosocial care to cancer patients and their families. Responsible for the psychosocial assessment planning implementation monitoring and support of patients through the continuum of their cancer care from diagnosis to survivorship or end of life. Using appropriate utilization of resources and maintain compliance. Utilizes independent critical thinking and collaborates with multidisciplinary team to address patients psychosocial needs.
Position is full time. Primary focus for this role is supporting medical oncology and radiation oncology patients. Will collaborate closely with two other Oncology LCSWs in the department.
Position Responsibilities:
Serves as the psychosocial service coordinator for the oncology program. Required to oversee all distress screening activities for the provision of psychosocial care which is a required component of the cancer program. To incorporate the screening of distress into the standard of care for oncology patients.
Identifies psychological social financial insurance spiritual and behavioral issues that may interfere with the patients treatment plans and adversely affect treatment outcomes.
Provides program development and coordination of survivorship care which is a required component of the cancer program. Responsible for development and implementation of cancer support groups.
Responds to clinical social work consults as referred by physicians professional staff and other team members. Administers suicidal assessments and crisis support in the outpatient setting.
Provides need based one on one psychotherapy sessions to individuals and their family members utilizing the following treatment modalities: Cognitive Behavioral Therapy Brief Solution Focused Therapy Narrative Therapy Gestalt therapy FamilyBased Therapy Emotionally Focused Therapy and Acceptance and Commitment Therapy.
Completes thorough assessment and mini mental status exam for all patients engaging in a therapeutic interaction.
Addresses any psychosocial issue that may present as a barrier to care throughout treatment utilizing research informed practice to increase patient functioning.
Follows NASW guidelines for clinical documentation while upholding confidentiality ensuring that record keeping adheres to state mandated procedures.
Facilitate referral upon completion of therapy to outpatient therapist based on clients insurance and location preference.
A required member of the multidisciplinary team and serves as a liaison between the patients/family and medical team to optimize psychosocial wellbeing and reduction of barriers to care related to cancer diagnosis treatment and survivorship and/or end of life care.
Coordinates evidencedbased care between patients and treatment team by advocating a holistic approach to every patient.
Facilitates communication among patient and families physicians nursing and other hospital employees through multidisciplinary rounds Oncology Care Conferences Oncology Practice Council Breast Leadership Program and Cancer Committee.
Assists patients and families in the areas of resource planning (including community support services fertility options housing transportation issues financial hardship spiritual needs childcare needs endoflife planning etc..
Assists patients and families in understanding healthcare benefits and maximizing decisions regarding coverage.
Provides guidance and instruction determining eligibility for Social Security Disability Social Security Income Charity Care Medicaid and other government assistance programs.
Assists patients in applying for oncology related grants to alleviate financial burdens. Acts as a liaison with community health and other social service agencies.
Member of the palliative care team which is an essential component of the cancer program and must be continuously available during treatment and in the bereavement period.
Administers distress screening to all new outpatient palliative patients and addresses psychosocial concerns.
Discuss the importance of Advanced Care Directives with patients and their families.
Develop and coordinate a 9 month Masters in Social Work student intern program. Clinical supervision and task supervision of MSW students for 22.5 hours per week per student.
Provide one hour of facetoface clinical supervision for each student weekly. Encourage guide and train students to function as independent oncology social workers in the outpatient setting.
Develop maintain and report out on oncology social work metrics and quality initiatives. Responsible for development of new oncology social work initiatives and supportive programs.
Required Experience:
Must be a Masters prepared and Licensed Social Worker.
Oncology hospice/palliative care and psychotherapy experience strongly preferred.
Case management and/or medical social work experience required.
Palliative Care Certification preferred.
Demonstrates competence in providing advanced clinical social work services including psychosocial counseling with individuals and families.
Excellent verbal and written communication skills problem solving organization and conflict resolution.
Ability to understand medical terminology and communicate effectively with persons of diverse backgrounds.
Required Education:
Minimum education of Masters Degree in Social Work.
Training / Certification / Licensure:
Current/valid State of New Jersey Social Worker License
Oncology Social Work certification preferred
Licensed Clinical Social Worker preferred (required to obtain within 2 years of hire)
