Home Care Hospice RN Case Manager 12 Week Contract
Position: Home Care Hospice Case Manager
Location: Colorado Springs CO
Duration: 12 weeks
Hours: Monday through Friday 8 AM 5 PM (No weekends)
Pay Rate: Up to $57 per hour

Job Description: Are you an experienced Hospice Case Manager looking for a rewarding contract position We are seeking a compassionate and skilled Home Care Hospice Case Manager for a 12week contract in Colorado Springs CO. Enjoy a Monday through Friday schedule with no weekends and earn up to $57 per hour while making a significant impact on patients lives.
Responsibilities:
- Manage and coordinate hospice care for patients in their homes.
- Develop and implement individualized care plans.
- Provide compassionate support and education to patients and their families.
- Collaborate with interdisciplinary team members to ensure comprehensive care.
- Document patient progress and adjust care plans as needed.
- Travel to patients homes using reliable transportation.
Requirements:
- Minimum of 2 years of experience in hospice care.
- Current RN license in good standing.
- Reliable transportation for home visits.
- Strong clinical skills and ability to manage complex care needs.
- Excellent communication and interpersonal skills.
- Compassionate and patientcentered approach to care.
