The LPN LTC is responsible for providing specialized care to patients in a longterm care setting. Working under general supervision this role assesses plans and implements interventions to promote optimal healing and prevent complications. This position collaborates with the interdisciplinary team to ensure the delivery of highquality care and adherence to best practices in wound management.

Responsibilities
Conducts comprehensive assessments of patients needs to develop individualized care plans. Performs treatments following established protocols practices. Monitors and evaluates the progress of patients documenting findings accurately and communicating any changes or concerns to the healthcare team. Educates patients families and caregivers on proper care techniques prevention strategies and signs of complications. Collaborates with physicians registered nurses and other healthcare professionals to coordinate care ensure continuity and optimize patient outcomes. Maintains accurate and uptodate documentation of assessments interventions and outcomes in the electronic medical record system. Performs other duties as reasonably required or assigned.
Qualifications

EDUCATION: Required: High School Diploma or equivalent; Diploma in Practical Nursing.
EXPERIENCE: Preferred: 1 year acute care experience
LICENSURE/CERTIFICATION/REGISTRY/LISTING: Required: Current LPN license in North Carolina; BLS (CPR)American Red Cross or AHA Healthcare Provider correct
