Job Description

BronxWorks helps individuals and families improve their economic and social wellbeing. From toddlers to seniors we feed shelter teach and support our neighbors to build a stronger community. BronxWorks is hiring one 1 Tenant Activity Specialist for the SHUD Scattered Site Supportive Housing Program.


RESPONSIBILITIES

  • Report to and meet with supervisor on a monthly basis
  • Coordinate variety of activities groups and events that cater to all ages of clients including children teenagers and adults
  • Collaborate with CQI staff case management and clinical staff to coordinate and run various events groups activities and workshops
  • Assist with bridging the gap between clients and property management by organizing tenant meetings and encouraging participation for tenant advisory council/board and community engagement
  • Assisting clients with ADLs such as managing finances shopping for groceries preparing meals and housekeeping (e.g. cleaning doing laundry)
  • Transport clients in agency vehicle for medical appointments grocery shopping program events and other locations as needed.
  • Carry a small caseload of up to 6 clients.
  • Develop and maintain connections with internal and external agencies and companies who can provide services and programs to clients
  • Liaise with case management staff to collaborate with them on clients programming needs and concerns and share relevant information to the case managers.
  • Maintain records of client interactions group meetings attendance and event menus in accordance with HUD DOH and Agency standards
  • Provide direct services to clients including intake engagement assessment service plan development referral advocacy counseling exit summaries and followup.
  • Document progress notes for all encounters and services provided to clients
  • Draft print mail copy fax and file memoranda letters flyers and other agency documents
  • Provide crisis interventions and manage emergency situations appropriately
  • Participate in regular supervision with supervisor
  • Conduct onsite outreach and advertising for planned events
  • Conduct home visits to clients as needed and actively engage clients who are absent or decrease their participation level and involvement in services
  • Attend department and agency meetings as required
  • Make ongoing assessments on what interests clients have
  • Maintain familiarity with agency and city resources available to clients
  • Complete reports monthly or more frequently as required
  • Creating and implementing a Needs Assessment for the clients to target gaps in care interests and needs of the clients
  • Assess the effectiveness of activities and make improvements based on data
  • Maintain and ensure accuracy of records and documentation of all client interactions
  • Actively engage the clients who are absent or stop participating in services by conducting appropriate outreach based on the residents needs to maintain continuity of services
  • Maintain organize and restock storage unit program supplies and materials
  • Perform additional duties as assigned

QUALIFICATIONS

  • Bachelors degree with a minimum of one years experience in social services and office administration
  • Candidate must have a valid drivers license which has been held for at least 1 year; in the last 3 years candidate must have obtained no more than 3 license violation points
  • Proficiency in English and Spanish preferred
  • Proficiency in modern business communications including personal computers electronic mail voicemail facsimile and copier equipment

PHYSICAL REQUIREMENTS

  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand walk or sit for long periods of time.
  • Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work.
