Job Description
SEEM Collaborative seeks to fill the position of Physical Therapist to provide inperson services to students inour member districts and our SEEM schools if needed.
Qualifications:
Masters degree preferred. Must be certified and registered by the National Board for Certification of Physical Therapists. Must have reliable transportation and be able to meet the essential functions of the job.
Scope of duties:
- Delivers physical therapy services in accordance with the students IEP in either a school or homebased setting.
- Consults with students teachers and/or caregivers as deemed necessaryby the districtbased team.
- Attends IEP meetings and other meetings as deemed necessary by the districtbased team.
- Provides written evaluations and reports at least on a quarterly basis and otherwise as required.
- Evaluates studentsphysical therapy needs and develop goals and objectives to be considered as part of the IEP process.
Employment is contingent upon passing a SAFIS fingerprint and CORI screening.
