Position Summary

The Home Visitor is responsible for the implementation of the New Beginnings Home Visiting Program. The New Beginnings is an evidence base program that provides parent education and supportive services to at risk families within the Chicago area. The program aims to assist families to develop skills and strategies to support the healthy development of their young children 03 years of age) children and families. The Home Visitor works with the parents in their homes providing early childhood education services in order for the parent to implement age appropriate activities and strategies to help the child develop their physical emotional cognitive language and communication skills. The Home Visitor also provides supportive services to the families and helps them obtain community resources.

RESPONSIBILITIES

  • Performs all work in a manner consistent with the National Youth Advocate Programs Mission Values and Philosophies.
  • Conduct outreach and recruitment efforts to identify eligible families for the program
  • Completes family intakes and family needs assessments and develop family service plans in conjunction with the family
  • Conducts weekly home visits to the families homes in accordance to program requirements.
  • Completes child developmental assessments based on the childs individual age
  • Ensures each assigned familys progress and services are documented and assessed. Maintains family record enters timely and complete case notes
  • Ensures all program records are maintained in a confidential manner and site reports are timely complete and accurate
  • Assist in the preparation of program reports
  • Obtains and maintains certification on required evidence based curriculum
  • Participates in weekly supervision with program manager
  • Performs all duties in accordance with personnel policies and fiscal guidelines
  • Perform other duties as requested by the supervisor

MINIMUM QUALIFICATIONS

  • Masters degree in Social Work Education or Psychology from an accredited school preferred. Bachelors degree required.
  • At least 3 years of professional experience working with low income at risk families and familiarity with evidence based intervention programs (EBI).
  • Trained on Parents as Teachers (PAT) curriculum preferred
  • Bilingual English/Spanish fluency required
  • Proficient use of desktop and laptop computers smart phones and tablets printers fax machines and photocopiers as well as software including word processing spreadsheet and database programs.

OTHER SKILLS

  • Demonstrate a high degree of customer service and relationship building abilities.
  • Excellent written and verbal communication skills.
  • Ability to work independently as well as part of an extended cross functional team.
  • Extensive travel within Cook and northern counties required. Access to reliable and insured vehicle required.
