Job Description
In collaboration with primary care physicians and other clinical staff members the Triage Registered Nurse is responsible for the provision of direct and indirect nursing care including physical psychological and social assessment which requires nursing judgment intervention referral or followup health maintenance teaching counseling planning and restoration for optimal functioning and comfort of the patients we serve within a PatientCentered Medical Home model. This position requires a high degree of flexibility independence and willingness to collaborate with patients and families to enhance the delivery of patient care services across the continuum of care.
