Job Description

Bulfinch Temporary Service is currently looking for a Nurse Practitioner to work at MGHs Transplant Department. The position begins ASAP and is for 40 hours per week. Please note this position is temporary in nature and not eligible for benefits. Under the supervision and direction of the Program Directors of the Lung Transplant Program with collaboration and oversight from the Transplant Center Sr. Administrative Director and in accordance with the rules and regulations of Massachusetts governing Nurse Practitioner practice the Nurse Practitioner (NP) will be responsible for ensuring the continuity of care of potential transplant recipients and living donors (as applicable) predominantly during the transplant and posttransplant phases however will also work collaboratively and may provide care to patients during the pretransplant phases of transplantation. The NP will serve as the central point of reference for all information about patients participating in the program. The scope of the position includes: patient care education quality assurance research administrative and financial activities and coordination of transplant care with medical and surgical staff as appropriate. A NP is a registered professional nurse with certification in an expanded role who provides direct care to individuals families and/or groups in a variety of care settings. In collaboration with physicians she/he is responsible for diagnosing prescribing treatment; and providing followup for patients during all phases of wellness and illness. She/he initiates referrals for implementing the patients total care plan involving other health disciplines within and outside the hospital. NPs place a marked emphasis on patient and family teaching and education. The NP may also assist in the OR.Principal Duties and Responsibilities: Clinical Practice Has a sound knowledge of nursing medical and scientific principles and appropriately applies those principles to practice. Provides independent primary nursing practice. She/he performs physical examinations institutes diagnostic studies prescribes treatment and medications and follows patients appropriately utilizing Clinical Guidelines as a basis for actions. Assists physicians with procedures in the OR as necessary. Performs medication management and reconciliation. Functions in direct collaboration with the physicians assigned to the Transplant unit. Initiates facilitates and develops interdisciplinary collaboration. Participates in all aspects of the transplant patients care including pre transplant while in the hospital and post transplant. In collaboration with patients and families she/he designs a plan of care based on an assessment of physical emotional and social needs. Identifies current trends in health care and their implication for nursing practice. Responds to changes in clinical practice by planning designing implementing and evaluating guidelines protocols and standards in collaboration with other professional staff. Completes discharge summaries and paperwork on patients a shared responsibility with the residents. Inputs outside hospital labs/studies into OnCall flowsheet. Follows up on transplant management labs results on patients for any tests performed without an associated outpatient clinic visit. Participates in multidisciplinary rounds and divisionwide conferences or other key transplant patient management meetings. Provides patient and family education throughout the patients care including but not limited to diabetes management. Provide support and clinical management to offservice transplant patients. For emergent issues and when surgeons are unavailable or in the operating room evaluate and manage patients and consult surgeons in the OR as appropriate. Applies innovative approaches to solving complex clinical problems. Assists with the coordination and booking of ORs inpatient admissions/preparations for transplants Triages phone calls from Transplant Clinic answering service during offhours Maintains patients right to privacy; assuring patient dignity in the provision of care. Transplant Referral and Evaluation (as applicable to Inpatient): Facilitate initial contact between referring MD or potential recipients and Transplant Center. This may include direct outreach to community medical practices. Identifies appropriate candidate upon referral and interacts with appropriate staff Identifies requirements for candidate evaluation Reviews tests during evaluation process and subsequent admissions in order to develop a care plan which may require additional testing or referrals to specialists/testing. Obtains a translator if needed Acts as a resource to the Transplant RN Coordinator when coordinating with the multidisciplinary team the clinical aspects of transplant patient and living donor care during the following phases: evaluation immediate pre and postoperative care discharge and outpatient followup. NP should play an active role in the evaluation of the patient both in the outpatient and inpatient setting. The evaluation should include but is not limited to: o Medical/surgical evaluationo Educational assessmento Laboratory evaluationo Radiological evaluationo Nutritional evaluation Interprets serological and other test results of the potential transplant candidate and their implications for transplant Identifies possible comorbidities in the transplant candidate and evaluates as indicated. Ensures that patient is assessed for presence of malignancy prior to listing Identifies and assesses absolute and relative contraindications to transplantation Ensures patient understands the risks and benefits of immunizations required prior to transplantation Functions as the candidate advocate by applying knowledge of growth development educational and cultural background during candidate/family/legal guardian interactions. Provides education to candidate/family/legal guardian about: o Living donation optionso Evaluation processo OPTN regulationso Organ allocationo Waiting phaseo Transplant processo Risks/benefits including utilization of expanded donors high risk donors if applicableo Postoperative phase including immunosuppressive therapy and effectso Potential research protocols of the transplant programo Candidate/family/legal guardian responsibility throughout the transplant process Identifies and responds to the psychosocial and economic needs of the transplant candidate/family/legal guardian during evaluation Collects and evaluates medical and psychosocial data for review by the transplant team Communicated with referring physicians other health care prociders payors and transplant team members regarding the candidates status and evaluation outcome. Informs and educates patient/family/legal guardian at the time of listing Ensures patient and referring physician are informed in writing of transplant listing denial or postponement Acts as the resource person for internal and external health care providers including patients and their family/support members Participate in the selection of patients for the program PreTransplant Waiting Period: Ensures cytotoxic antibody screen is obtained regularly per transplant center protocol Facilitates admission or transfer of patient to the transplant facility Arranges any preoperative procedures required (e.g. cardiac catheterization biopsy) Exhibits ability to place patients on the waitlist change the status of patients already on the waitlist and remove transplant recipient from list per OPTN/UNOS regulations Maintains communication with referring physicians health care providers and payors Orders tests/referrals as appropriate and acts as a resource for internal and external health care providers Assists in the assessment and management of the patients chronic lung disease or heart failure as they await a donor organ. Transplant Phase Facilitates admission date with donor and transplant candidate and notifies transplant team members of pending surgery Coordinates needed consultation and testing for transplant recipients and live organ donors throughout the admission/donation phase. Working with the multidisciplinary team including the staff nurses identifies and coordinates response to the educational psychosocial and financial needs of recipients and live organ donors and families during the admission/donation phase. Maintains/ensures communication with recipients/donors primary physician and other health care providers throughout the procedure and postoperative recovery Documents multidisciplinary round participation in the patient medical record (On Call) Facilitates the discharge phase PostTransplant Discharge Phase Provides care within his or her scope of practice and licensure in the outpatient setting to achieve optimal physical social and emotional rehabilitation. Identifies and responds to recipient/family/legal guardians educational psychosocial and economic needs in the posttransplant phase. Reinforces education and followup requirements necessary for successful outcome. Observes monitors and assesses recipients compliance with medications and medical regimen; collaborates with the transplant team to attain and maintain compliance. Collects and reviews data pertinent to recipients health and organ function and ensures that the information is documented in the patients chart/OnCall. Reviews lab data and diagnostic results and makes appropriate care plan changes as applicable Manage changes in drug/treatment. Understands and carries out posttransplant protocols and acts as a resource for internal and external health care providers. Arranges for consultations diagnostic procedures and hospitalization when indicated. Maintains communication with referring physician and other health care providers. Provides and maintains comprehensive documentation in patient medical record. Identifies and responds to the educational psychosocial and economic needs of transplant recipients/live organ donors and families during the posttransplant/followup phase. Administrative and Financial Participate in developing implementing and evaluating on an ongoing basis guidelines protocols and standards. Maintains system of recordkeeping on all potential transplant candidates and liver organ donors and transplant recipients and organ donors. Is knowledgeable in regard to requirements for documentation data submission quality assurance and other reporting of UNOS NEOB CMS and other regulatory bodies and ensures compliance with Policy and Procedure for Data Submission to the OPTN. Assists in the maintenance of monthly and annual statistics including summaries of clinical data and postoperative followup care. Collaborate with the Transplant Financial Coordinator (TFC): notification of TFC of patient referral for transplantation; communicate with the TFC with regard to patient eligibility for transplant benefits. Serves as a clinical resource to third party payers. Participates in/provides data for site visits requests for information etc. of third party payers UNOS CMS and other regulatory bodies. Collaborates among MGH organspecific transplant programs to ensure efficient and effective use of resources and expertise. Initiate and participate in required quality assurance activities. Participate on committees and task forces that relate to organ transplantation as requested by Physician Director(s) and/or Transplant Center Director (or designee). Maintain data in clinical records and databases as appropriate to evaluation/clinical care and consistent with Transplant Center/organspecific policies and procedures Relationship ManagementMaintain relationships with new and existing referring physicians and their clinical and office staff. Serves as a liaison and facilitator of communication between physicians administration and community. Identifies and monitors relationship histories including details of prior referrals from referral sources and referrers preferred communication methods by maintaining information in the OnCall database. Identifies and summarizes referring physician/center/clinician needs to the Heart and Lung Transplant physicians. In conjunction with Heart and Lung Transplant physicians and leadership develops standards for communication with referring physicians/centers/clinicians and works with Heart and Lung Transplant physicians and leadership to implement. Actively communicates patients progress and discharge plans to referring physician and assure that communication is maintained throughout admission at discharge and after outpatient visits. Followsup on all requests to ensure satisfied patients and referring physicians. Assists in family liaison education and postdischarge planning. Patient Relations and Program Support Develops systems to ensure followup with patients referring physicians and internal transplant team members. Receives routine clinical questions or updates from patients whether by phone or email. Reviews patient questions with physician when appropriate and responds. Documents all patient conversations or emails in the electronic medical record systems. Participates as requested in practice divisional MGH and Partners quality initiatives. Participates in various hospital and system committees as assigned. Documents and reports all referring physician questions concerns complaints and resolutions. Meets routinely with transplant physicians and leadership to report on activities referral patterns customer satisfaction problems and recommendations. Meets routinely with supervisors to review statistics progress and overall performance of the program. Educational Develops and implements patient instructions for routine testing and procedures. Acts as a resource to other care providers with questions about cardiothoracic transplantation and its care management. Assists with ongoing clinical research through followup of patients currently involved in transplant clinical trials. Professional Development Maintains and updates knowledge and skills based on current education/nursing and health care practices. Is encouraged to advance his/her medical and technical skills as appropriate to deliver optimum patient care. Identifies own learning needs and goals and designs a plan to meet them. Participates in professional staff and student teaching both formal and informal. Fulfills professional role through involvement in professional organization activities presentations publications and research. Attends internal and external education meetings and reads appropriate professional journals in order to maintain and enhance skills and clinical competence. Completes the 100 hours of continuing medical education necessary during each 2year period of employment. Skills/Abilities/Competencies Required: Demonstrates maturity selfconfidence.
